Objective To monitor the changes of rennin(REN), angiotensin-Ⅱ(AT-Ⅱ), aldosterone(ALD) at the different time during retroperitoneal laparoscopic surgery and open sursery. To probe the effect of CO2 pneumoperitoneum on rennin-angiotensin- aldosterone system during retroperitoneal laparoscopic surgery.Methods To select 20 patients underwent the urinary retroperitoneal laparoscopic surgery and 10 patients underwent the urinary open surgery in department of urology of the first affiliated hospital of Anhui medical university from Aug. 2008 to Feb. 2009. There were 19 males and 11 females , 1878 years old averaged age 45.2±9.8. There were ureterolithotomy 3 casas, nephrectomy 8 cases, renal cyst decompression surgery 9 cases in retroperitoneal laparoscopic surgery group (RL) and radical nephrectomy 2 cases, Anderson-Hynes 2 cases, polycystic kidney decompression surgery 2 cases, pyelolithotomy 3 cases , nephrectomy 1case in open surgerg group (OP) . The REN ,AT-Ⅱand ALD levels in blood plasma of all patients were observed by RIA respectively at 10 mins(T1) before insufflation, 30 mins(T2) and 60 mins(T3) after insufflation , 60 mins(T4) after desufflation in RL and at 10 mins(T1) before surgry , 30 mins (T2) and 60 mins (T3) in surgery , 60 mins(T4) after surgery in OP. The experimental data (χ±s) were analyzed by using a statistical soft system SPSS 13.0.Results There were the significant changes of the REN ,AT-Ⅱand ALD levels in blood plasma of all of the patients at the different time during retroperitoneal laparoscopic surgery . There were T3>T2>T4>T1, but no significant difference (P>0.05) betwe T4 and T1. The changes of OP were similar. The degrees of the changes of REN ,AT-Ⅱand ALD levels at different time in RL were no significant difference compared to those in OP.Conclusions CO2 pneumoperitoneum during retroperitoneal laparoscopic surgery could affect rennin-angiotensin- aldosterone system. But the effect would dissolve after desufflation soon. The degree of the effects in RL was no significant difference compare to those in OP.
