Objective Artefiosclerosis Obliterations is a kind of common disease in vascular surgery. With the prolongation of the life and especially the improvement of diagnosis and examination, the therapy of ASO has been improved quickly. This article is to explore the combined Endovascular and Extra-vascular treatment of ASO.Methods To summarize 30 cases of patients of ASO who were treated with Endovascular and Extra-vascular therapy in the Affiliated Qian-Fo Shan Hospital of Shandong University from March 2007 to June 2009, the combined treatment of Endovascular and Extra-vascular were performed on 36 limbs in 30 patients suffering from lower extremity multilevel atherosclerotic occlusive disease. In 36 limbs endovascular treatment were operated with simultaneous femoral artery angioplasty in 3 limbs, various types of bypass in 19 limbs and femoral artery embolectomy in 14 limbs. Results Surgical procedures were technically successful in all patients, In addition to three cases of incision occurred in patients with lymph leakage (via medication therapy cured),There were no severe postoperative complications. Before-operative vs postoperative ABI was 0.33±0.18 vs 0.72±0.13 (P<0.05). The following-up period range from 3 to 48 months. Follow-up observations found that the patient s symptoms were significantly improved,3 patients had residual intermittent claudication (claudication distance of 700~1000m), 2 patients occurred secondary artificial vascular thrombosis due to graft intimal hyperplasia. Conclusion The combined treatment of Endovascular and Extra-vas cular are effective in the treatment of patients with arteriosclerosis obliterans. The therapy has a flexible surgical method and which can effectively relieve symptoms of limbs, The operation is less traumatic for patients and the procedures are easy to operate for doctors. The operative method is especially suitable for high risk patients.
