Object1.To observe and compare the therapeutic effect of focused ultrasound and budesonide nasal spray in the treatment of perennial allergic rhinitis.2.To analyze the efficacy of the surgical treatment for allergic rhinitis patients with nasal septum deviation and nasal inferior turbinate hypertrophy;and to compare the effect of different surgical methods.3.To analyze the allergen types and distribution of perennial allergic rhinitis patients with moderate to severe in Liaoning Province.4.To analyze the rhinitis-related quality of life in perennial persistence allergic rhinitis patients with moderate to severe, To evaluate the correlation between different indicators in rhinitis-related quality of life,To compare the quality of life improvement in different treatment methods before and after treatment.MethodsTo divide the 65 patients who are all meet the inclusion criteria into two parts by nasal structural abnormalities. The first part (20 patients) of the nasal cavity was normal and they were receiving focused ultrasound therapy as the operation group. Choose equal number of patients selected as the drug group to accept budesonide nasal spray treatment at the same period. Observation period is 3 months. Record symptom score, VAS score and the visual quality of life score. The second part, the patients (45 patients) accompanied with deviation of nasal septum, inferior turbinate hypertrophy. Divided them into three groups, A:To correct deviated septum, relocate the inferior turbinate and partial excision; B:To correct the deviation of nasal septum, under a double inferior turbinate focusing ultrasonic therapy; C:To correct the nasal septum relocate and excise the inferior turbinate and take a double inferior turbinate focused ultrasound therapy. To complete the 6-month follow-up, record symptom score. All selected patients were made of skin prick test (SPT) in the clinic, and preliminary analyze the allergen inspection results. Admission, follow-up, record the quality of life score, and analyze the results.Results1.In normal nasal structure patients, the score in the operation group are better than drugs group in every time of postoperative follow-up.2.At the patients accompanied with deviation of nasal septum, inferior turbinate hypertrophy, three different surgical methods all get a significantly improve after 6 months than before surgery, C group (To correct the nasal septum relocate the inferior turbinate and partial excision under a double inferior turbinate focused ultrasound therapy) get the best. B(To correct the deviation of nasal septum, under a double inferior turbinate focusing ultrasonic therapy), nasal blocking entry get poor. A (To correct deviated septum, relocate the inferior turbinate and excision) in sneezing, nasal discharge, nasal itching symptom scores were not as good as on the B group and C group.3.39 cases of skin prick test results:dust mites19 cases,20 cases of house dust mites, ragweed 16 cases,19 cases of mugwort, pollen in 1 case, cat hair in 2 cases,2 cases of German cockroaches, mold in 1 case.4.Correlation analysis of the quality of life:the lack of good sleep and nasal block symptom score was significantly correlated; sleep and non-nasal total score are significantly correlated with symptoms of conjunctivitis; the lack of good sleep and work efficiency decreased significantly correlated; The distribution of symptoms of allergic rhinitis and the level of impact on daily activities result:work, social activities, vacuum cleaners clean the room, followed by doing housework is the most selection, daily activities, mowing, raise pets, walking, followed by vacuum cleaning the room is to choose the highest scores of daily activities.Conclusions1.compare with drug therapy, Focused ultrasound treatment have a rapidly onset, short-term effect is significant.2.allergic rhinitis patients accompanied with nasal cavity structural abnormalities, surgical treatment can significantly improve symptoms; but focused ultrasound on the effect of reduction in capacity have a less effect; To correct the nasal septum relocate the inferior turbinate and partial excision under a double inferior turbinate focused ultrasound therapy make a best results.3.Dust mite is the top allergens in Liaoning Province.4.Correlation analysis showed that nasal symptoms may be the main reason affecting the quality of sleep, sleep quality can affect the non-nasal-conjunctivitis symptoms.
