PART ONE ESTABLISHMENT OF SHEEP MODEL OF ALLERGIC RHINITIS SENSITIZED WITH TOLUENE-2, 4-DIISOCYANATEObjective:To establish animal model of allergic rhinitis with large animal which have spacious nasal cavity and discuss its practicability.Methods:1, 4 Nanjiang sheep were executed and nasal dissected. Anatomic parameters were recorded. 2, 12 Nanjiang sheep were enrolled in the experiment, 8 in model group and 4 in control group. 15% Toluene-2, 4-Diisocyanate (TDI) olive oil solution was used as sensitizer in model group and olive oil solution was used in control group. Symptoms and sings of sheep were recorded during modeling. After last challenge, the level of histamine in nasal lavage fluid was measured and nasal mucosa was histopathology examined. 3, Sheep which had been successfully sensitized were divided randomly into group A (Budesonide nasal spraying agent treatment) and group B (normal sodium as control). Differences between pre and post treatments were recorded and compared to evaluate the therapeutic efficacy.Results:1, Nanjiang sheep have similar nasal anatomic structure with human being. 2, Compared with control group, all the sheep in model group got typical symptoms and sings of allergic rhinitis after TDI challenge, the level of histamine in nasal lavage fluid was highly increased, differences were both statistically significant (P<0.01). Histopathology examination of nasal mucosa showed obvious sub mucous tissue edema and vasodilatation, inflammatory cell scattered or focal infiltration on lamina propria (mainly eosinophile). 3, Symptoms and sings of sheep in group A released after budesonide nasal spraying agent treatment, the difference compared with group B was statistically significant (P<0.05).Conclusions:Allergic rhinitis animal model of large animal was successfully established. With multi-methods proved, the model could be used for allergic rhinitis drug effect research, as well as safety and efficiency assessments for allergic rhinitis physical and operative therapeutics. PART TWO OBSERVATION OF CLINICAL EFFICACY AND EXPLORATION OF MECHANISM IN TREATING PERENNIAL ALLERGIC RHINITIS WITH HIGH-INTENSITY FOCUSED ULTRASOUNDObjectives:To observe the clinical efficacy of High-intensity Focused Ultrasound in treating perennial allergic rhinitis, try to explore its treatment mechanisms from immunological aspects.Methods:1, 143 patients with perennial allergic rhinitis which met the diagnostic criteria were treated by High-Intensity Focused Ultrasound. Score their symptoms and sings before the operation and the first month, sixth month, twelfth month after the operation. Contrast the scores to evaluate their clinical efficacy. 2,Randomly select 30 cases from the patients as treatment group, 20 healthy adults as control group. Before the operation and the sixth month after the operation, the following indicators of the two groups were measured: 1, Nasal local immune indicators: HE staining to show histopathological changes in nasal mucosa; Fluorescence spectrophotometer to detect histamine content changes in nasal mucosa. 2, Peripheral blood of systemic immune indicators: Fluorescence spectrophotometer to detect histamine content changes in peripheral blood; Flow cytometry to detect changes in the proportion of th1 and th2 cells in peripheral blood.Results:1,The total clinical effective rates after high-intensity focused ultrasound treatment were: the first month97.9%; the sixth month89.5%; the twelfth month: 81.1%. 2,When the sixth month after the operation, in the 30 cases of treatment group there were 27 patients who show effective. Their histopathology examination showed the nasal mucous membrane were integrity, and the sub mucous tissue edema and vasodilatation, inflammatory cell scattered had been greatly improved; The content of histamine in nasal mucosa and in peripheral blood were 1.93±0.75ng/mg and 83.56±16.92ng/ml; The th1 cell and th2 cell percentages were 19.22±6.39 and 2.49±1.05; The ratio of th1 and th2 cells was 10.04±0.38. The differences compared with pro-operation were statistically significant (P <0.01).Conclusions:The total clinical effective rate of High Intensity Focused Ultrasound in treatment of perennial allergic rhinitis was high. The treatment mechanisms may be related to the changes of the local and systemic immune response state after treatment.
