PART ONE THE BASIC STUDY FOR THE FOCUSED ULTRASOUND IN THE TREATMENT OF ALLERGIC RHINITIS: INFLUENCE OF ULTRASOUND ON PERITONEAL MAST CELLS DEGRANULATION OF SD RATOBJECTIVE: Mast cell as an extremely sensitive degranulated cell is the main cell which involved in the allerg reaction of allergic rhinitis. However, degranulation of the MC will release a series of inflammatory mediators and cytokines, will result in heavier patients or nasal allergy symptoms, is not conducive to the treatment of AR. This experiment to study the possibility of ultrasound-induced SD rat peritoneal mast cells (rat peritoneal mast cells, RPMC) isolated in vitro degranulation and to explore its mechanism, and analysis of the feasibility of HIFU treatment of AR.METHODS: Female SD RPMC were isolated and collected after decapitating , ultrasonic radiation in two, the determination of cell activity was observed in two cells under the conditions of acute death of whether there will be no radiation group died of acute MC in scanning electron microscope to observe the changes in cell surface morphology, and fluorescence spectrophotometry for determination of mast cell histamine release rate.RESULTS: After the ultrasonic irradiation of CZB-therapeutic rhinitis apparatus, MC inⅠ-Ⅳfiles are within the scope of acute death .No RPMC acute death was observed after 30 second ultrasound wave irradiation in energy parameter of 1.0MHz,2W/cm2.and on the 30th second, histamine release rate was the highest. With scanning electron microscope, RPMC particles prolapse and minipores in cell membrane were observed. Computation shows dose-dependent manner of RPMC histamine release rate in certain energy parameter. And its comparison with control group shows statistical significant difference (P<0.05).CONCLUSIONS: CZB-therapeutic rhinitis apparatus can kill suspended MC directly.Ultrasound can induce degranulation of isolated SD RPMC, and its mechanism might concerned with cavatition erosion. PART TWO THE CLINICAL STUDY FOR THE FOCUSED ULTRASOUND IN THE TREATMENT OF ALLERGIC RHINITIS: COMPARED THE THERAPEUTIC EFFECT BETWEEN FOCUSED ULTRASOUND AND MICROWAVE TREATMENT OF PERENNIAL ALLERGIC RHINITISOBJECTIVE: Focused ultrasound and microwave treatment of perennial allergic rhinitis (Perennial Allergic Rhinitis, PAR) are physical therapies. This experiment to compare the clinical therapeutic effect between Focused Ultrasound and microwave treatment to PAR and to investigate the advantages of the former.METHODS: 219 cases of PAR patients randomized, 143 were treated with CZB-therapeutic rhinitis apparatus treatment, 76 cases in the control group using microwave therapy apparatus treatment. The therapeutic effect was evaluated by clinical symptoms controlled counting-scores, according to the Chinese Lan Zhou principle of treatment and recommendation program for allergic rhinitis (2004).RESULTS: A total of follow-up after 12 months, with a total effective rate are as follows: 1month: 97.9% in the treatment group and control group, 94.7% (P> 0.05); 6 month: 89.5% in the treatment group and control group, 85.5% (P> 0.05); 12month: 81.1% in the treatment group and control group, 68.4% (P <0.05).CONCLUSIONS: Focused Ultrasound treatment of perennial allergic rhinitis, a more lasting effect, pay attention to the protection of physiological function of the nasal cavity, safe, after light reaction and rapid recovery, is a worthwhile promotion of physical therapy.
