ObjectiveTo study the changes of serum visfatin and hs-CRP in patients of type 2 diabetic mellitus (T2DM) and the association of visfatin with MAU(microalbuminuria). To investigate the clinical role of Visfatin in the pathogenesis of vascular endothelial dysfunction, insulin resistance and type 2 diabetes mellitusMethodsAccording to the standards of diabetic diagnosis and typing put forward by WHO in 1999, a total of 60 patients with T2DM were selected. Based on albumin creatinine ratio (UACR) of real-time urine samples, the patients with type 2 diabetes were divided into two groups:30 patients with normal albuminuria and 30 patients with microalbuminuria.30 healthy volunteers from conventional physical people were enrolled as control.. All subjects were measured fasting Visfatin, fasting hs-CRP, fasting blood glucose, fasting insulin, blood lipid, HbAlc, urine albumin urine creatinine and BMI, HOMA-IR were calculate. The data were ananlyzed by ststistical software SPSS 13.0 version.Results1. The serum Visfatin, hs-CRP level in patients of type 2 diabetes were higher than normal control group (16.85±4.61 vs 14.21±3.82和0.82±0.12 vs 0.70±0.10, P<0.05)2. The serum Visfatin, hs-CRP level in patients of type 2 diabetic with microalbuminuria were significantly higher than patients with normal albuminuria (23.5±5.36 vs 16.85±4.61,1.22±0.16 vs 0.82±0.12, P<0.01) and normal control (23.5±5.36 vs 14.21±3.82,1.22±0.16 vs 0.70±0.10, P<0.01)3. A correlation analysis showed that there were remarkable possitive correlation between Visfatin and hs-CRP, UACR, HOMA-IR, HbAlc, TC and no correlation between Visfatin and FBQ BMI, WBC, TQLDL,HDL.4. Multiple stepwise regression analysis showed that Visfatin remained the independent influence factors for UACR in type 2 diabetic patientsConclusionThe level of serum Visfatin in patients of type 2 diabetes elevated, the level of serum Visfatin in patients of type 2 diabetes with microalbuminuria increased significantly. Furththermore, Visfatin positively correlated to hs-CRP, HOMA-IR. As a pro-inflammatory cytokines, these results indicated that relations between Visfatin and inflammation. Visfatin may take part in the development of vascular endothelial dysfunction and insulin resistance in patients with T2DM.
