Clinical Effect Observation Of Treating Central Obesity With Electric Needle Acupuncture Plus Catgut Embedding

ObjectiveCentral obesity is the independent risk factor for cardiovascular diseases. Its disease danger and lethal danger are higher than general obesity. This randomized controlled study takes electric needle therapy as contrast and makes objective evaluation of the effect of applying catgut embedding plus electric needle acupuncture to treat simple obesity.MethodsThe research objects are 110 qualified central obesity patients who were treated in Taiwan clinics from February to October 2009. Major acupoints of the Treatment Group (electric needle plus catgut embedding):Belt pulse, Zhongwan, Tianshu, Shuifen, Fenglong. Supplementary acupoints:for Spleen Deficiency and Dampness Stagnation, plus Spleen Shu, Yinlingquan acupoints; for Stomach Heat and Dampness Stagnation, plus Quchi and Foot Sanli; for Liver Depression and Qi Stagnation, plus Liver Shu and Foot Sanli; for Spleen and Kidney Deficiency, plus Guanyuan, Spleen Shu and Kidney Shu; for Yin Deficiency and Internal Heat, plus Sanyinjiao and Kidney Shu; for constipation, plus Fujie, Shangjuxu and Foot Sanli. Acupoints are selected at both sides. Catgut embedding procedures:tools:take type-8 injection needle as the needle tube, take 2-cun fine needle of Huatuo Brand with the diameter of 0.3 mm as the needle core, and take catgut of type 000. Operation:Ask patients to take the comfortable position. The doctor sends the catgut of 1 cm to the needle body from the needle tip of type-8 injection needle (at the same time, the fine needle inside the injection needle should withdraw a little bit) and the thread should be even with the inner part of the needle tip. Sterilize the skin of the acupoint. The doctor should tighten patients'acupoint skin with the left hand and insert the needle quickly into the acupoint with the depth of 1.5cm to 2.0 cm. Then pull the fine needle inwardly and withdraw the injection needle slowly so as to leave the catgut inside the patient's body. Examine whether there's thread revealed. Stick the strapping to the pinhole for 24 hours. Electric needle acupuncture operations:Routine sterilization of the acupoint and select type-30 acupuncture needle of 40-60mm (sterilized acupuncture needle of Huatuo Brand) according to the patients'obesity degree. Insert the needle quickly with single hand. Lift, twist and rotate the needle so as to achieve qi. Apply type-G6805 Electric-needle Treatment Device and apply the wave of 2/100HZ which should be bearable for patients. Then maintain the needle for 30 minutes. The Comparison Group:Acupoints selected are same as those of the Electric Needle Acupuncture Group.ResultThis randomized controlled clinical research takes 110 qualified subjects with 55 in the Treatment Group (catgut embedding and electric needle acupuncture) and 55 in the Comparison Group (electric needle) who are all clinic patients.Comparing each index of the two groups before treatment, the author finds the difference has no significance which indicates there's comparability between the baseline materials of the two groups.Curative effect comparison:There's significance of difference in comparing the Chinese Medicine syndromes. The Treatment Group's curative effect is superior to that of the Comparison Group. The Treatment Group's clinical recovery rate is 23.64%, the excellence rate is 40.0%, the effective power is 21.82% and the total effective power is 89.09% while those of the Comparison Group are 12.73%,27.27%,40% and 85.45% respectively. Analysis of the two groups'clinical curative effect with rank sum test (z=2.02, P=0.04) shows the difference has statistical significance. Theχ2 test of the total effective power of the two groups after treatment finds that the difference has statistical significance.After a treatment course, compare the two groups and find the difference in the changes of waistline, TC, TG, HDL-C, and LDL-C has statistical significance while the difference in BMI has no statistical significance. After two treatment courses, the difference in the changes of BMI, waistline, TC, TG, HDL-C, and LDL-C has statistical significance. The interclass comparison shows that the difference in TC, TG, HDL-C and LDL-C has no statistical significance.Appetite improvement degree after treatment:After two treatment courses, the comparison of the appetite improvement degree of two groups (z=2.12, P=0.033) finds that the difference has statistical significance. The difference of the disappearance rate of debility after treatment has significance. Compare the disappearance rate of the complaints including breath shortness, lassitude, drowsiness and excessive appetite and polyphagia, and find the difference has no statistical significance. There's significance in the disappearances rate of the abdominal distension after treatment. There's no statistical significance in the disappearance rate of complaints like constipation, striae atrophicae, drowsiness, pale purple striae, over-sweating and afraid of heat.Conelusion1. Electric needle plus catgut embedding is superior to electric needle therapy in the curative effect of improving the Chinese Medicine syndromes.2. Electric needle plus catgut embedding is superior to electric needle therapy in lowering BMI and improving the biochemical indexes.
