ObjectiveThis clinical controlled study discusses the clinical effect of acupuncture and makes objective evaluation of applying warm needle and electric needle acupuncture to treat periarthritis of shoulder.Methods68 patients of periarthritis of shoulders are chosen who meet the Chinese Medicine and Western Medicine diagnostic standards and who were treated in Ming De Tang Chinese Medicine Clinic, Taiwan from March to December 2009. With randomized controlled methods, the author classifies them into Treatment Group (Warm Needle Group) and the Controlled Group (Electric Needle Group) at the rate of 1:1 with 34 patients in each group. Acupoints selected are Jianyu, Jianzhen, Jianliao, Ashi, Hand Sanli, Waiguan, and Houxi. Treatment course: three time a week and totally 4 weeks. The observation index is chosen according to the evaluation scale of shoulder joint function. The curative effect is determined according to the Chinese Medicine Disease Diagnostic and Evaluation Standard issued by State Administration of Chinese Medicine. At the same time, safety index is measured.Collect the clinical material in time after the clinical research. Establish a database with software Epi Data 3.01 and analyze with software SPSS 17.0. Statistics methods:The classified material is processed with X2 test. The ranked material is processed with Wilcoxon rank sum test. The comparison of the average of the two samples takes t test or Wilcoxon rank sum test. The self circa comparison takes pair t test or Wilcoxon pair rank sum test. The statistical graphs are made with software Graph Pad Prism 4.03.ResultThe research results are listed as follows: 1. The clinical curative effect of periarthritis of shoulder:The Treatment Group's clinical recovery rate is 44.1%, the excellence rate is 26.4%, the effective power is 17.6% and the total effective power is 88.3% while those of the Controlled Group are 23.5%,23.5%,35.2% and 82.4% respectively. By rank sum test, comparing the curative effect of the two groups, the author finds the difference has statistical significance (P<0.05). The Treatment Group's general curative effect is superior to that of the Controlled Group. Compare the total effective power of the two groups with chi square test, the author finds that the difference has no statistical significance (P>0.05). It indicates that the two groups both achieve good curative effect and warm needle can raise the recovery rate and the excellence rate.2. Pain index score:The scores of the Treatment Group and the Controlled Group rise from 48.21±6.48 to 68.62±4.93 and from 47.34±5.12 to 62.56±3.77 respectively. The comparison of the two groups before and after treatment with t test shows that the difference has statistical significance (P<0.01) The comparison of the two groups after treatment and the alteration values with t test shows that the difference has statistical significance (P<0.05) and the Warm Needle Group has the better curative effect. These prove that the two groups can both greatly release patients'pain while warm needle acupuncture has better analgesia effect.3. Shoulder joint motion function score:The scores of the Treatment Group and the Controlled Group rise from 13.12±1.43 to 20.21±1.38 and from 12.66±1.37 to 17.34±1.32 respectively. The comparison of the two groups before and after treatment with t test shows that the difference has statistical significance (P<0.01). The comparison of the two groups after treatment and the alteration values with t test shows that the difference has statistical significance (P<0.05) and the Warm Needle Group has the better curative effect. These prove that the two groups can both improve patients' clinical symptoms and improve the function of shoulder joints while warm needle acupuncture has better curative effect than the electric needle acupuncture.4. Shoulder joint motion range:After treatment, patients of two groups have better ability of lifting, stretching, retroflexion and external rotation (P<0.01). The difference of the curative effect of the two groups is significant (P<0.01). The improvement of the Warm Needle Group is better than that of the Electric Needle Group. It indicates that warm needle acupuncture can better improve the shoulder joint motion range.ConclusionUsing warm needle to treat the periarthritis of shoulder is simple, economical, effective and safe and reliable. It's superior to electric needle therapy in raising the recovery rate, release pain, and improve the function of shoulder joints, therefore it's worth of being further recommended in clinical treatment.
