The Relationship Between The Cardiac Function Of ESRD Patients And The Radial Augmentation Index

Objective:To investigate whether the radial augmentation index(AI) is related to intima-media thickness(IMT) and abdominal aortic calcific-ation(AAC) which were considered as the predictors of cardiovascular diseases in maintenance hemodialysis(MHD) patients.Then we determine the AI and the cardiac function of those patients,to identify if there were relationships between them so as to predict the incidences of cardiovascular diseases and improve the prognosis of the MHD patients earlier.Methods:Twenty-eignt MHD patients were included in the present study.They were 18-65 years old,and their mean HD age was 50.04±9.74 months.The patients who were in the acute or chronic inflammation state,having the lower blood pressure (<90/60mmHg),arhymia,both arms were operated by the internal arteriovenous fistula were excluded.Detect the serum calcium,phosphate,PTH, hemoglobin. AIX 75bpm was evaluated by HEM-9000 AI type(Japan omron corporation) pulse wave detection device,whose heart rate was corrected by 75 times per minute. IMT, left ventricular end diastolic dimension(LVDd),interventricular septual(IVS), ejection fraction (EF), left ventricular posterior wall thickness (LVPW), the left ventricular diastolic dysfunction indicators E/A,and left ventricular mass index (LVMI) were evaluated by color doplor ultrasonography. Calcification of the abdominal aorta above its bifurcation was evaluated semi-quantitatively in 10 CT slices at 1 cm interval.Anlyze the relationship between AI and IMT,AAC, cardiac function parameters such as LVDd,IVS, EF,LVMI,E/A,LVPW.Pearson's correlation coefficients were used to analyze the association of clinical parameters. A probability value of P <0.05 was considered to be statistically significant. Results:(1)The AI of the MHD patients showed positive correlation with IMT (r=0.407,P<0.05) and AAC (r=0.428,P<0.05), IMT showed positive correlation with AAC (r=0.530,P<0.05).(2) The AI of the MHD patients showed positive correlation with LVDd,IVS, EF, LVMI, E/A, LVPW.Conclusions:(1)The AI of the MHD patients showed positive correlation with IMT,AAC,and IMT,AAC showed positive correlation with each other (2)The radial AI could be considered as one of the indices of cardiovascular diseases in MHD patients.
