| Background and objective:Fractional laser treatment is based on fractional photothermolysis theory. By targeting water in tissue as a chromophore, laser divided into micro laser induces microscopic treatment zones in skin. With energy absorbing by chromophore in treatment zones and epidermal repairing by activating surrounding keratinocytes, collagen fiber proliferates, synthesis of collagen protein increases, consequently skin quality improves.It is used to heal photoaging epidermal diseases, pigment diseases, various scars (including acne scars), rhytides in face and neck, or other epidermal diseases.In present study, we observed the curative effects and side-effects of fractional laser on treatment of acne scar, pore enlarging, rhytides, and melasma.Methods:Fifty three patients (10 males and 43 females) with acne scars (44 cases), pore enlarging (53 cases), rhytides (27 cases), and melasma (7 cases) were observed. Patients were irradiated with a 1550-nm Fractional laser in 0.46ms of pulse width,10-30mJ of energy,100-529ppa of energy density,312-1000μm of light spot interval,0.2-1.5mm in pen-etration depth, with contact cooling. Evaluation criterion:Ineffective, as grade 1, means improvement<29%; Effective, as grade 2, means impro-vement 30%-69%; Excellent, as grade 3, means improvement〉70%. Total effective rate equals the summation of effective rate and excellent rate. Pain was evaluated by 10 grades score. Zero means totally painless while 9 means acute needling-like pain.Results:Forty-four patients with acne scars got total effective rate as 65.91%, and average clinic improvement grade as 1.75.53 patients with pore enlarging got total effective rate as 67.92%, and average clinic improvement grade as 1.83.27 patients with rhytides got total effective rate as 59.26%, and average clinic improvement grade as 1.70.7 patients with melasma got total effective rate as 100%, and average clinic improvement grade as 2.14.Average pain grade in operation is 3.6(1-8), pain grade after operation is 4.2(1-8) within 0.5-3 h. Post-treatment skin erythema lasted 3-7 days, edema persisted in 1-2 days, bronzing pigmentation existed in 3-5 days, skin whitening occurred around 7 days after scales shedding of skin.Conclusions:1. After 3 times of fractional laser treatment, satisfactory curative effect were achieved in improvement of acne scar, pore enlarging, rhytides, and melasma.2. The side-effects of fractional laser treatment are mainly temporary response such as pain, erythma, edema, and flaking. |
