Objective:Renal cell carcinoma(RCC) is the second largest urinry cancer. It is arising trend in the incidence in recent years.Early symptoms of RCC is not obvious.Their metastasis mainly through lymphatic and hematogenous.However,the current reatment of RCC is radical surgery,but have high of recurrence .The results of radiotheraphy and chemotherapy are poor,needing for other effective treatment.The discovery of molecular genetics,to find new and effective method of treating RCC,provide theoretical basis in recent years.The occurrence,development and metasta- sis are retremely complex process,Which involoved many tumor-associated factors participate and control,pathogenesis is not clear.In recent years hypoxia-inducible factor-1α(HIF-1α)may play an important role in tumor cell proliferation,apoptosis,angiogenesis,invasion and metastasis. chemokine receptor(CXCR4)as only receptor of strornal cell derived factor-1(SDF一1).Studies suggest that CXCR4 has an important role in rumor-specific metastasis. Clear cell renal cell carcinoma(CCRCC)is the most common pathological type of RCC.This study investigated that the erpression and clinical significance of HIF-1αand CXCR4 in CCRCC in CCRCC.It may be provide a theoretical basis of diagnosing and treating for RCC.Methods:Collected 48cases from patients with surgical specimens and Randomly selecte 200cases normal renal tissure from 48 cases.All spe- Cimens were fixed in formalin and they had been confirmed by pathology. None of the cases for radiotherapy and chemotherapy. The expression of HIF-1αand CXCR4 was checked in 48 cases of CCRCC and 20 cases of normal renal tissues on the protein level by Immunohistochemical methed.The correation between them and clinical stage,pathological grade, lymphatic metastasis ,age,sex and tumor size were analyzed.Results: The HIF-1αand CXCR4 expression in CCRCC tissue were 58.3%,77.0%,there were barely detectable HIF-1αand CXCR4 protein in nomal renal tissue.There were more significant differences.(P<0.01).InⅠ,Ⅱ,ⅢCCRCC Fuhrman histological grade, HIF-1αprotein expression were 37.5%,52.6%,92.3%,the higher with increase of pathological grade, there were more significant differences(P<0.05). InⅠ,Ⅱ,ⅢCCRCC Robson stage, HIF-1αprotein expression were 38.9%,61.9%,88.9%,the higher with increase of pathological stage, there were more significant differences(P<0.05). In the groups with and without lymph nodes metastases,the positive rate were respectively,the difference between the two groups was significant(P<0.05). CXCR4 had close correlation with clinical stage, pathological grade and lymphatic metastasis(P<0.05), HIF-1αand CXCR4 was not related to the patientage,sexuality and tumor diameter(P>0.05). HIF-1αwas positive correlative with CXCR4(P<0.05).Conclusion: HIF-1αis highly expressed in the CCRCC, and with pathological grade ,stage, lymphatic metastasis, suggest that HIF-1αis closely related to tumor proliferation, apoptosis, invasion and metastasis infiltration. It can be used as indicators of progress and It may serve as an independent factor that predicted survival rate of patients with CCRCC. CXCR4 might promote the occurrence and development of CCRCC,which has a high expression in CCRCC and plays a promote role in the transfer. Detecting CXCR4 is expected to become CCRCC early diagnosis and monitoring of CCRCC marker and It has important significance in the treatment and prognosis .
