Objective: To detect the dynamic variation of the serumα-MSH and TNF-a in multiple injury patients and discuss the relation between it and inflammation in disease variety of injury patients. Methods: 50 patients with injury were divided into two groups in the study,30 patients were middle and severe patients (16-25point); the other 20 patients were especially severe patients (25 point); 15 person were normal healthy. We observed the changes ofα-MSH and TNF-a after the multiple injury at 8 o'clock of 1days,3days, 5days, 7days in the morning when extracted venous blood and compared them with normal persons'. After the serum separated, enzyme-linked immunosorbent double antibody sandwich method (ELISA) was applied to evaluate the levels ofα-MSH and TNF-a in multiple injury patients. Results: 1.The serumα-MSH level within 1days, 3days, 5days, 7days after injury between the two groups were much lower than those in the control group(P<0.01). 2.The serum TNF-a level were much higher than those in the control group(P<0.01). 3.There was significant difference (P<0.01)at l,3,5,7d and negative correlation at l,5,7d on the indexes above of the two groups.4. The levels ofα-MSH in the serum of multiple injure patients with MODS were lower than those of the without MODS at l,3,5,7d.The difference was significant between the two groups (P<0.01 and P<0.05). but The levels of TNF-a in the serum of multiple injure patients with MODS were higher than those of the without MODS at l,3,5,7d.The difference was significant between the two groups(P<0.01 and P<0.05). 5.The levels ofα-MSH in the death group were lower than those of the survival group at l,3,5,7d.The difference was significant between the two groups(P<0.01 and P<0.05).but the levels of TNF-a in the death group were higher than those of the survival group at l,3,5,7d.The difference was significant between the two groups (P<0.01 and P<0.05).Conclusions: In trauma patients , the amount ofα-MSH decreased , the amount of TNF-a rose , and the trauma was more severe , the change was more significant. There was a negative correlation between both of the indexes in general. The detection of the serumα-MSH and TNF-a is helpful in evaluating the possibility of multiple injury with MODS and accurate evaluating the illness and prognosis in time.
