Objective This study aims to explore a quick detection method with high sensitivity, specificity and no invasion through combined detection of ultrasound, palpation and PSA concentration in serum.Method 3 ml venous blood was drawn from 317 patients from January of 2009 to August of 2010 at urinary surgery in Shanxi province tumor hospital, and the detection of PSA concentration in serum and ultrasound were carried out.Results The results showed that the range of PSA concentration in serum with age from 30 to 40 years old was mainly around 0-4ng/ml. There was 67.3% with PSA concentration around 0-4ng/ml for the patients with age from 50 to 60 years old, However, for the patients with age from 61 to 70 years old the same PSA level was 38.5%,4~10ng/ml with a proportion of 30.75%,10.1-30ng/ml with a proportion of 16.9%,30.1-50ng/ml with a proportion of 10.7%, larger than 50.1ng/ml with a proportion of 3.1%. With age larger than 61 years, PSA level around 0-4ng/ml was 38.5%, which was significantly lower than the patients'with age younger than 60 years, but for PSA level larger than 10.1ng/ml, Compared with the younger than 60 years, the proportion of PSA level with age larger than 61 years was higher. According to the diagnosis criterion about prostate carcinoma (PSA>4ng/ml), this study showed that the detection of PSA concentration in serum could play an important role in the earlier detection of prostate carcinoma with different age patients.Among 317 patients, there were 116 patients which were pathologically confirmed prostate carcinoma and 201 patients with prostate benign disease (prostate hyperplasia, prostate inflammation). With PSA level in serum,The positive rate was 62.46%(198/317), the sensitivity was 70.68%(82/116), specificity was 66.67%(134/201); with abdominal ultrasound and transrectal ultrasound, The positive rate was 88.79%(103/116),which was larger than 70.68%(the positive rate of PSA level in serum).The combined detection of ultrasound and PSA concentration in serum could increase the sensitivity up to 92.24(107/116), and the specificity up to 89.05 %(179/201), which was higher than the PSA examination and ultrasound examination respectively, with a statistically significant difference between two groups (P<0.01).Conclusion The study showed that the PSA concentration has high related with age. With age increase, PSA level will increase accordingly, so the positive results should be depended on different ages. For people with age older than 60 years, PSA level should be examined every year.The combined detection of ultrasound and PSA concentration in serum could increase the sensitivity and the specificity, which was higher than the PSA examination.Compared with ultrasound examination, the combined detection of ultrasound and PSA concentration in serum increase the specificity, but there was no statistically significant difference.The combined detection of ultrasound and PSA concentration in serum was applied for the earlier screening of prostate carcinoma, which could increase the sensitivity and the specificity, at the same time the side effect brought by prostate biopsy could also be avoided.
