Objective To explore the clinical diagnostic value of prostate cancer with transrectal color Doppler ultrasound (TRUS) and 6+X-point prostate biopsy under the guiding of TRUS. Methods(1)88 cases were involved in our study. They were planned to undergo prostate biopsy because of serum PSA more than 4ng/ml and positive finding by digital rectal examina-tion positive and/or TRUS.The positve finding of transrectal color Doppler included abnormal echo nodules and blood flow.45 patients with serum PSA between 4ng/ml and 30ng/ml were selected,and the prostatic volume of every patient was examined with TRUS. Then PS-AD of every patient was producted according to serum PSA and prostate volume.The differ-ence of PSAD between the group of prostate cancer and the one of benign prostatic hyperplasis or prostatitis was analysed with t test. The diagnostic value of prostate cancer with PSAD was compared with the one with serum PSA.(2)88 cases were performed TRUS-guided biopsy systems 6-point,subsequently the suspected focuses-called X point was carried out addi-tional biopsy. The positive ratio was compared between 6 point prostate biopsy and 6+X point prostate biopsy. Results(1)The Ultrasonogram performance of the group of prostate cancer was significantly different with the Ultrasonogram performance of the group of internal nodular hyperplasia(P<0.05).The detection ratio of PSAD was higher significantly than that of ser-um PSA among patients with serum PSA between 4ng/ml and 30ng/ml.TRUS sensitivity was74.7%,specificity was68.9%,positive predictive value was 69.6%.(2) All cases were pathologically confirmed,including 41 cases of prostate cancer,41 cases of benign prostatic hyperplasia,1 case of prostatitis,3 cases of benign prostatic hyperplasia combined with pros-tatitis,1 cases of atypical hyperplasia,and 1 cases of prostate tuberculosis.The detection rate of prostate cancer through 6+X point biopsy system was 46.6%,sensitivity was 95.3%,spe-cificity and the positive predictive value were both 100%.41 cases of prostate cancer patients such as only 6 points by biopsy to have 6 cases of missed diagnosis of patients,the misdiagno-sis rate was 14.6%.Conclusions(1)The lesions of prostate can be clearly observed through transrectal color Doppler,TRUS had a important role in the differentiation of malig-nant prostate diseases from benign prostate diseases.PSAD was a more sensitive index com-pare with serum PSA among patients with serum PSA between 4ng/ml and 30ng/ml.(2)6 +X-point biopsy method under the guiding of TRUS can improve the detection rate of pros-tate cancer,and the method is simple,safe,accurate and had few complications. It had a good significance in the terms of the diagnosis and the selection of the treatments of prostate cancer.
