| ObjectivesThe article aims to compare the variation of various Traditional Chinese Medicine (TCM) patterns of chronic heart failure with the normal control group and explore the correlation between the change of thyroid hormone level and syndrome patterns and meanwhile it may provide certain reference for the disease conditions.Methods201 CHF patients was selected who meet the inclusion criteria and determine the TCM pattern of them by using four diagnostic methods, and collecting basic information on patients, as well as the past medical history, clinical symptoms, such as tongue, pulse signs of clinical data. Patients' TCM pattern was determined according to《The clinical research governing principle of the New herbal drug》and patients'clinical symptoms, tongue and pulse signs. The 201 cases have been divided into seven groups, there are Heart and lung Qi deficiency pattern, Qi and Yin deficiency pattern, Qi deficiency and blood stasis pattern, heart and kidney yang deficiency pattern, yang deficiency with edema, phlegm and fluid retain in the lung and yin and yang exhausted simultaneously pattern. LVEF was measured by an experienced cardiovascular B-ultrasound doctor via colorful Doppler ultrasound equipment. The 201 cases of above-mentioned group with heart failure and 30 cases of control group was chosen as the subjects and the T3, T4, FT3, FT4, TSH, rT3, Tg, TGAb, and TPOAb was measured with radioimmunoassay method. Data has been gathered and the Observation Schedule was disposed and therefore the data base was established. The SPSS 13.0 statistical software was applied to conduct the statistic analysis and all the measurement data was present as mean士standard deviation. The t-test and variance analysis was applied for the measurement data and x2 test was applied for the enumeration data. P<0.05 indicates that the difference has statistical significance.Result1.Compare the chronic heart failure of different Traditional Chinese Medicine pattern with the heart functional classification. And the heart functional classification has statistic significance of each pattern(P<0.05). which indicates the heart function fade out:Qi and yin deficiency pattern---heart and lung Qi deficiency pattern---phlegm and water retained into the lung pattern---Qi deficiency and blood stasis pattern---Heart and Kidney Yang Deficiency pattern---Yang deficiency with edema pattern---both Yang and Yin exhausted pattern.2. When compared between thyroid hormone level of chronic heart failure group and its normal control group, the chronic heart failure group of FT3 level has been reduced significantly, and it has significant difference (P<0.05). rT3 rose significantly when compared with the normal control group and there is significant difference (P<0.05).The comparison of level of T3 T4, FT4, TSH, Tg, TGAb, TPOAb of these two groups indicates no significant difference. When compared the thyroid hormone level of the chronic heart failure of different TCM patterns with normal control group. And there is significant difference between the levels of the T3 decreasing level and its normal control group.3. The level of FT3 decreased by the following order,Qi and yin deficiency patternheart and lung Qi deficiency pattern---phlegm and fluid retained in the lung pattern---heart and kidney Yang deficiency pattern---Yang deficiency with edema group---and Yang and yin exhaustion pattern. Among the above group, the heart and Kidney yang deficiency, yang deficiency with edema group, and the Yang and yin exhaustion pattern decreased significantly. When compared with the other four patterns, there is significant difference (P<0.05), and there is no significant difference among the level of each group of T4, FT4, TSH, TG, TGAB, TPOAb. The level of the rT3 rose gradually by the above order and the most significant groups are Yang deficiency with edema and Yin and Yang exhaustion. Meanwhile, these two groups have significant difference when compared with the normal control group (P<0.05).There is no significant difference among different groups of the level of T4, FT4, TSH, Tg, TGAb, and TPOAb(P>0.05)4. The reducing level varies among different heart function classification, and there is significant difference (P<0.05).When comparing the rT3 rising level of the heart functionalⅣgroup with that of the heart functionalⅡgroup, there is significant difference. The Other measurement indicators have no significant difference among different groups.Conclusion1. The blood serum FT3 level of the CHF patients has been reduced significantly. There is significant difference (P<0.05); The level of rT3 have increased significantly compared with the normal people. There is significant difference (P<0.05).2. The CHF patients centre with kidney Yang deficiency, Yang deficiency with edema, exhausted Yin and Yang pattern has been uprising apparently. Compared with the normal group, these two groups have significant difference (P<0.05).
