Clinical Obersevation Of Qindan Drink On Improving Urinary Albumin Of Diabetic-nephropathy Patients

ObjectivesThis study aims to observe the effect of Qindan drink on the excretory rate of urine protein of patients with diabetic-nephropathy and to observe the clinical efficacy of Qindan drink for diabetic-nephropathy patients.MethodsThis research refers to the diagnostic standard of diabetes in the Chinese Prevention Guide of Diabetes (2007 version) and the Chinese and Western Medical Diabetes and its complications (1997 version, published by People's Medical Publishing House), in order to diagnose theâ…¢-â…£phase DN. And it also refers to Diabetes Guidelines for clinical research of TCM in Guiding Principles for New Drug Clinical Research of TCM (2007 version), citing 90 cases of inpatients and outpatients with diabetic-nephropathy with syndrome of yang deficiency of spleen and kidney, who were divided into three groups:treatment group, control group and combined group,30 patients for each. Patients from every group were on strict diet and in exercise therapy; insulin was regularly used to control their blood sugar level. The treatment group took Qindan drink, one dose per day, once each morning and evening,200ml at a time; the control group took fosinopril, once per day, 10mg at a time; the combined group took 10mg fosinopril once per day and Qindan drink, one dose per day,200ml each time both in the morning and evening. The course of treatment lasted for two months. The blood sugar level of all the groups was controlled to 130/80mmHg; those who couldn't reach the standard, took extended release felodipine tablets. Those who had abnormal blood-lipid took Xuezhikang twice a day. The different clinical symptoms of patients before and after the treatment and the improvement of their physical signs were under observation; also their changes of blood-lipid level, Scr, BUN, CysC and excretory rate of urine protein before and after, were monitored. By comparing the data, we could draw a conclusion on the efficacy of Qindan drink in curing DN.Results1 In terms of improving clinical symptoms, the total effective rate was 90% in the treatment group, and 93.3% in the combined group, both were significantly higher than that of the control group, which was only 60% (P<0.05).2 The excretory rate of urine protein of the three groups all declined, the control group (P<0.05), the other two groups (P<0.01). But compared with the control group, the other two groups greatly surpassed the control group (P<0.05). But comparing the treatment group and the combined group showed no statistic significance (P>0.05).3 The BUN, Scr and CysC of the three groups all decreased after the treatment (P<0.01), which showed a significant statistic difference. But compared with the control group, the other two groups had a significantly different reduction (P<0.05). But comparing the treatment group and the combined group showed no statistic significance (P>0.05).4 The LDL-C, TC and TRIG of the three groups all decreased significantly (P<0.01). After the treatment, the TC, LDL-C, TRIG level of the treatment group and the combined group had a statistic difference from that of the control group.ConelusionApplication of Qidan drink according to differentiation of symptoms can significantly decrease the excretory rate of urine protein of diabetic-nephropathy patients. Qidan drink can also improve clinical symptoms; it can ameliorate TC, LDL-C and TRIG quickly and manifestly, and ameliorate renal function. Qidan drink is an effective cure for diabetic-nephropathy patients.
