Objective:To study the visual field of diabetic retinopathy in different course of disease and changes of the urinary albumin excretion rate, discussing the the relationship between the TCM syndrome type of diabetic retinopathy and the visual field and urinary albumin. Methods:According to the method of multicenter clinical research, divide the DR patients who meet the Inclusion criteria of the standard of 1987 ophthalmic institute of Chinese Medical Association into groups, the observed will be examined by general condition, ophthalmologic and UARE examination, then fill in CRF which Including general symptom, self-conscious symptom, tongue and pulse, capillary pathological changes staging, renal damage condition and laboratory biochemical indicators, also will be examined by OCTOPUS 101 for the white perimetry of the center 0~52°and use the D1 diabetes visual field test procedures. At last, put all data into data base and analyze them by statistics.Result:①As the aggravate of DR condition, the MS has a gradual decline, MD,LV has a gradual rise trend; the MS of the visual field of PDR patients is decreased obviously than preclinical and NPDR patients(P<0.01); the MD of the visual field of PDR patients is increase obviously than preclinical and NPDR patients(P<0.01); the LV of the visual field of PDR patients is increase obviously than preclinical and NPDR patients(P<0.01).②Spearman related analysis shows that DR staging negatively correlated with MS, DR condition is heavier, MS is smaller; DR staging positively correlated with MD, so the DR condition is heavier, MD is bigger; DR staging positively correlated with LV, the DR condition is heavier, LV is bigger.③Urinary albumin excretion rate is positively correlated with DR staging (P< 0.05),negatively correlated with DR,DM course (P>0.05),and unrelelated with MS,MD,LV.④TCM symptoms total credit is positively correlated with DR staging, as the aggravate of DR condition, TCM symptoms credit increase obviously.⑤TCM symptoms credit is negatively correlated with MS(P<0.05); positively correlated with MD (P<0.01), and unrelelated with LV.⑥CM symptoms total credit is highly positively correlated with DN stagin(normal albuminuria group, microalbuminuria group and macroalbuminuria group)(P<0.01), positively correlated with urinary albumin excretion rate(P<0.05).⑦he Vision index MS of DR patients is negatively correlated with blood deficiency syndrome(P<0.05),MD is positively correlated with blood deficiency syndrome(P< 0.05) and unrelelated with the syndrome of qi deficiency, yin deficiency, blood stasis, LV is unrelelated with the syndrome above, the UAER of DR patients is highly positively correlated with the syndrome of yang deficiency and blood stasis(P<0.01),unrelelated with the syndrome of qi deficiency, yin deficiency, blood stasis.Conclusion:①he visual function has damaged before retinal capillary pathological changes of the diabetes patients, as DR illness aggravating its visual field damage also gradually deteriorated.②he patient acuity of DR is closely related to the DR visual field damage and microalbuminuria excretion rate.③DR is closely related to DN, but Severity of the two does not completely parallel.④the TCM symptoms of DR patients Can reflectthe visual field damage of DR patients and Kidney damage situation.⑤he visual field of DR patients is correlated with Blood deficiency syndrome, the UAER of DR patients is highly correlated with the syndrome of yang deficiency and blood stasis.
