Background:The etiology of inflammatory bowel disease(IBD)still remains incompletely understood, but much more convincing and further studies have been done,and it is generally agreed that IBD result from an inappropriate innate and acquired immune response to commensal microorganisms in genetically susceptible individuals.IBD is systemic disease of the immune system that menifest themselves primarily in the gastrointestinal tract. IBD includes two major types of chronic intestinal disorders: Crohn's disease and ulcerative colitis. In Asia,the annual incidence of UC and CD are 1/10,000~2/10,000 and 0.5/10,000~1/10,000, respectively. Their prevalence are 4/10,000 ~44.3/10,000 and 3.6/10,000~7.7/10,000. IBD patients have two peak onset period around 15 years old and 30 years old。The medical dates of pediatric IBD are quite limited.With the improvement of many new technology , such as laboratory test and endoscopic technology,more and more IBD have been correctly diagnosised in childhood。So the epidemiological and clinical data of pediatric IBD need to be complete,and more and further clinical studies on the diagnosis and treatment of pediatric IBD。 Object:This study aim to find the clinical characters of pediatric IBD and summarize the experences of diagnosis and treatment, by evaluating the diversity of clinical manifestations , laboratory test , radiological,endoscopic and histological examination.Methods:1. Research object: First collect medical cases of all the mpatients whose discharge diagnosis considered inflammatory in Children's Hosipital of Chong Qing Medical University during December 2000 to December 2010;then we evaluated the cases again by the criteria instituted by Chinese Medical Gastroenteric Organization in 2001,the patients fit the criteria are enrolled,and also make a summary of the suspect ones。2. Methods: After retrospective analysis of the data of the cases,we get the current clinical and treatment situation of the patients via telephone,letters,asking them come back to the hospital and take some tests。3. Statistical analysis Then we separated them into different groups according to the clinical data, and finished the statistical analysis by the Statistical software SPSS 18.0 and Microsoft Office Excel 2003.The qualitative data was tested by fisher's exact test,and the quantitative data were expressed by median (M) , the range of the date, and tested by two-sample t-test,P < 0.05 means that there is a statistically significant difference.Result:The main intestinal and extraintestinal manifestations of IBD patients include:abdominal pain,diarrhea,hemafecia;and fever,anaemia,arthropathy,aphthous ulcer。As limited by the number of the cases,we have prove that there are statistical significant differences between UC group and CD group on clinical manifestations(P>0.05)。And we also have not found that there are statistical significant differences between the two group on the rout laboratory tests。There are statistical significant differences between UC group and CD group on the endoscopic appearance(P>0.05)。The lesion of UC much often present in the left part of the colon(P=0.027),while in the upper gastrointestinal tract (P=0.028),terminal ileum or jejunoileum (P=0.002) are more common seen in the CD group。On Biopsies,typical histological signs are detected in resection specimen much more frequently in biopsy samples。Conclusion:Abominal pain,diarrhea,hemafecia,fever,anaemia,arthropathy and aphthous ulcer are the most common clinical manifestations of pediatric IBD. blood routine test,CRP,ESR are important on monitoring the disease activity。Endoscopy play an irreplaceable role on the diagnosis of pediatric IBD,and capsule endoscopy need a further promotion in pediatrics。Although there are significant individual difference,the patients can be treated general well if insist on Regular treatment and follow up Procedures。...
