Objective: By detecting plasma lipoprotein associated phospholipase A2(Lp-PLA2) in patients with atherosclerotic cerebral infarction (ACI) and TIA, to explore the relationship between Lp-PLA2 and ischemic cerebrovascular disease . Further more ,to explore the relationship between the severities of neurological impairment in patients with atherosclerotic cerebral infarction (ACI)to evaluata tha severity of disease. Methods: Choose the 91 cases of acute cerebral infarction and 35 cases of transient ischemic attack for study object in inpatients or outpatients in the Department of Neurology of the Second Hospital of He Bei Medical University, from June 2009 to 2010 June. Meanwhile, choosing randomly 31 cases (no cerebrovascular disease risk factors) from medical examination center as the normal control group. After taking historyof no history of cerebovascular disease ,confirmed by CT or MRI without stroke. Inclusion criteria is the diagnostic criteria of the Fourth National Nonference for cerebovascular disease. TIA patients stroke recover completely within 24 hours, and there is no related responsible lesions on CT and MRI. Plasma level of LP-PLA2 were detected by enzymelinkedimmunosorbentassay(ELISA). The degree of neurological impairment for the patients with ACI were assessed acording to the National Instituteof Health Stroke Seale ( NIHSS seore ).All measured values were analysed by the 13.0 SPSS software. Results: 1 The levels of LP-PLA2,FIB,CHOL,TG ,HDL ,WBC and NEUT gradually increased in normal controls,TIA group and ACI group, the differences had statistical significance (P < 0.05).2 The plasma level of LP-PLA2 were higher in ACI(47.697±23.865 ug/L) than that of the TIA(31.973±22.078 ug/L), the differences had statistical significance (t=3.500,P=0.001).3 The more serious neurologic impairment the higher levels of plasma LP-PLA2 , the differences had statistical significance among groups (F = 7.119, P = 0.001). The mild and moderate were compared with the severe cerebral infarction by SNK-q test , the differences had statistical significance( F=21.047,P=0.004;F=16.576,P=0.029). The differences between mild and moderate cerebral infarction groups were no statistical significance.4 The plasma level of Lp-PLA2 was negatively correlated with HDL(rs=-0.337,P=0.000),positively correlated with CHOL,LDL(r_s=0.455,P= 0.000; r_s=0.484,P=0.000 ) , and was no correlation with FIB,TG,WBC,NEUT,age(r_s=0.139,P=0.120;r_s=0.145,P=0.106;r_s=0.143 ,P=0.110;r_s= 0.149,P= 0.096).Conclusions: 1 The plasma levels of Lp-PLA2 in ACI and TIA were higher than that in normal control group. In addition,The plasma level of Lp-PLA2 in ACI was higher than that in TIA. LP-PLA2 may be a new risk factors in TIA and ACI.2 Lp-PLA2 was correlated with the degree of neurological impairment for the patients with ACI.Therefore ,the severity of the disease was accessed by detecting the the plasma levels of Lp-PLA2.3 In the patients with ischemic cerebrovascular disease, LP-PLA2 was negatively correlated with HDL ,positively with CHOL, LDL, and no correlation with FIB and TG, NEUT and age.
