| Objective To Explore the relationship between the age,prostate volume,serum prostate specific antigen(PSA) and the acute urinary retention(AUR) in patients with benign prostatic hyperplasia(BPH),and evaluate their clinical application value in predicting the AUR with BPH. Methods Retrospective analysis the general clinical datum of 608 patients diagnosed BPH from January 2005 to December 2009 in Urology ward in the Affiliated Hospital of Ningxia Medical University,and divide the 608 cases into two groups according on whether they have the symptom of AUR,the"A"group have 323 cases without the symptom of AUR and the"B"group have 285 cases with the symptom of AUR.To compare whether the differences between the group about the age,prostate volume,T-PSA,free prostate specific antigen(F-PSA),F/T-PSA,maximum urinary flow rate(Qmax) and postvoid residual volume are significant,we analysised the dependablity among the age,the prostate volume and the T-PSA. The SPSS 11.5 software is adopted in statistical treatment.We adopted the t test to treat the datum between the two groups,and adopted the chi-square test to compare the count data,the linear correlation analysis was used in analysising the relation between the age,the prostate volume and the T-PSA. Results (1) In the group of without AUR and the group of AUR,the mean age were respectively (69.16±6.58) years and (70.64±7.08) years,the mean prostate volume were respectively (54.94±30.21)ml and (67.24±34.11)ml,the mean serum T-PSA were respectively (5.024±6.564) ng/ml and (8.241±8.245) ng/ml. After statistical treatment,there were all statistically significant differences between the two groups in age,prostate volume and serum T-PSA(P<0.01).(2) On the age,prostate volume and serum T-PSA,there were all statistically significant differences in different age groups,prostate volume groups and serum T-PSA groups (P<0.05).(3) In the group of without AUR and the group of AUR,there were statistically significant differences between the two groups in the maximum urinary flow rate, postvoid residual volume and F-PSA (P<0.01).But there weren't statistically significant differences between the two groups in the history of dysuria and F/T-PSA (P>0.05).(4) In the BPH patients,the prostate volume was positively correlated to the age (P<0.05),the T-PSA was positively correlated to the age (P<0.01),the T-PSA was positively correlated to the prostate volume (P<0.01).Conclusions (1) With the development of age,prostate volume and PSA,the chance of the AUR is increasing.So the age,prostate volume and PSA are the risk factors of AUR with BPH,and they can be used as predicting index of AUR.(2) The prostate volume is increasing gradually with the age,the PSA is gradually increasing,too;and the rising PSA is mainly due to the increasing prostate volume.(3) The maximum flow rate, postvoid residual volume and F-PSA are also the risk factors of AUR with BPH,but the history of dysuria and the F/T-PSA are not correlated to AUR with BPH. |
