Objective: Ameloblastoma is a usually frequently encountered jaw bone center tumor arising from odontogenic epithelium. It is characterized by benign tumor but locally invasive behavior with a high risk of recurrence. And it is prone to malignant tumor. So it is usually called borderline tumor. We study the expression of membrane type-1 matrix metalloproteinase and extrinsic inhibition (RECK) in ameloblastoma and investigat the genesis, development and invasive biologic behavior of ameloblastoma to apply theory basis to the preservation, diagnosis and therapy of ameloblastoma.Methods: There were collected sixty cases of ameloblastoma and thirty cases of odontogenic keratocyst. The immunohistochemistry SP method was used to study the expression of MT1- MMP and RECK in ameloblastoma and odontogenic keratocyst. And the statistical method was also used to analyze the clinicopathological characteristics such as gender, age and pathology.Result: In the thirty cases of odontogenic keratocyst, there were twenty cases (66.67%) with positive expression of RECK and three cases (10.00%) with strong MT1- MMP expression. In the sixty cases of ameloblastoma, 44 cases (73.33%)showed positive MT1- MMP reactivity and lower in primary ameloblastomas than in recurrent ameloblastomas. The expression of RECK was weakly reactive in ameloblastoma and the expression rate was 30.00%. In our study, the expression of RECK and MT1- MMP in ameloblastomas was significantly correlated with recurrence of ameloblastoma(P<0.05)but not with sex and age of the patients(P>0.05). RECK had a negative relationship with MT1- MMP in protein expression in AB(r=-0.634 ,p<0.001).Conclusions: The low expression or abscence of RECK and the positive expression of MT1- MMP played a significant importance in invasive biologic behavior of ameloblastoma and was related with the recurrence of ameloblastoma. RECK post-transcriptional inhibited the activity of MT1- MMP and regulated MMP-14 paticipating in the invasion and recurrence of ameloblastoma..
