Objective:Based on the previous study, on the one hand, reseach the clinical comprehensive intervention effects of invigorating the spleen and kidney,detoxification and vigoration of blood circulation based on Tangshenning cooperating to Chinese patent drug and auriculotherapy on early diabetic nephropathy(DN). On the other hand, explore the effect of Tangshenning on Flt-1 expression on rat models of diabetic nephropathy.Method1. Clinical Research:Based on relevant Standards of the Ministry of Health, tentative diagnosis and treatment of diabetes -induced kidney injury evaluation criteria were set up. 124 patients were randomly divided into two groups. One group was treated by using Tangshenning. The other one was treated by using Yikai(Pancreatic Kinionogenase Enteric-coated Tablets). They were clinically observed once for 8 weeks by using the primary indexes of symptoms, kidney function, lipid, and the secondary indexes of blood sugar, insulin, HbA1C, hemorheology.2. Experimental Research:rat model were made with high fat diet, STZ and single nephrectomy. The rats were randomly divided into the following 5 groups (n=10), the model group, the low-dose Tangshenning group, the high-dose Tangshenning group and the Fosinopril-Sodium group. After 12 weeks of drug intervention, rats were killed and the kidney of was removed, ultrastructure of kidney was observated by transmission electron microscopy. Its pathological structure changes were observed by using light microscope. Besides, their expression of Flt-1 were measured by using immunohistochemical method.Method 1.Clinical Research:Tangshenning can not only effectively improve symptoms and signs, kidney function (P <0.05), lower blood glucose and lipid levels (P <0.05) of the patients with DN, but also reduce insulin resistance, which the total effective rate was 90.6%, and It was significantly better than Yikai group.2 . Experimental Study: Light and electron microscope displayed that fatty degeneration in Tangshenning treatment group was less than model control group,especially in the high-dose group of Tangshenning. Besides,The effect of Tangshenning on the expression of the Flt-1 in model group was significantly decreased than control group (P <0.01). and Tangshenning group compared with the model group, Flt-1 expression was more increased (P <0.01),which with a dose related (P<0.05). Fosinopril-Sodium group compared with the model group was more pronounced (P <0.01), but it was weaker than Tangshenning treatment group (P <0.05).ConclusionTangshenning which was based mainly on invigorating the spleen and kidney, detoxification and vigoration of blood circulation can prevent DN. The mechanism is a multi-target, multi-mechanism, and deserves further research.
