Clinical Study Of Invigorating The Spleen And Kidney And Activating Blood Circulation Decoction On Oxidative Stress And Microinflammation Of CKD

ObjectiveThe purpose of this study was to investigate the clinical efficacy of invigorating the spleen and kidney and activating blood circulation decoction in patients with the stage III-IV of chronic kidney disease(CKD)of spleen and kidney deficiency with blood stasis syndrome.The changes of oxidative stress and microinflammation status indicators were observed before and after treatment.TCM syndrome scores,renal function and safety indexes were evaluated to further evaluate the relationship between oxidative stress and microinflammation status and disease development in CKD patients.To provide new treatment ideas and methods for the treatment of the stage III-IV of CKD,and then lay the foundation for the subsequent promotion.MethodAccording to the single-blind randomized controlled clinical research method,80 patients with the stage ?-? of CKD who were received treatment in the Clifford Hospital and met the syndrome of spleen and kidney deficiency with blood stasis were screened according to the inclusion and exclusion criteria while in October 2017 to October 2018.They were randomly divided into experimental group and control group,40 cases in each group.The control group received basic treatment,incl.uding high-quality low-protein diet,antihypertensive,hypoglycemic and other treatments.In the experimental group,the above-mentioned basic treatment was taken,and the spleen and kidney-activating blood was added daily.The treatment time of the above two groups was 4 weeks.After the end,the statistical comparison was made according to the relevant data before and after treatment.The contrast nodes of interest were:oxidative stress and microinf'lammation status indicators,TCM syndrome score,renal function and safety indicators,comprehensive evaluation of the clinical efficacy of invigorating the spleen and kidney and activating blood circulation decoction in the treatment of CKD.Result1.In TCM syndrome scores,before treatment,there was no significant difference between the two groups(P>0.05).There was a clear difference in TCM syndrome scores between the two groups after treatment,(P<0.01).About the experimental group the TCM syndrome scores were obvious difference before and after treatment(P<0.01);For the control group,there was a significant difference in TCM syndrome scores before and after treatment(P<0.01).Compared with the control group,after treatment,the efficacy of the experimental group was clear different(P<0.01);90.00%of the total effective rate of the experimental was significantly higher than that of the control group which got 27.50%(P<0.01);2.Before treatment,there were no significant differences in Scr,BUN,Alb,24h urine protein,eGFR and the stage of CKD between the two groups(P>0.05),and they can be comparable.There were significant differences in Scr,BUN,Alb,eGFR and the stage of CKD between the two groups after treatment.(P<0.01).Between the two groups,there were significant differences in 24h urine protein(P<0.05).There were significant differences in Scr,BUN,Alb,eGFR and the stage of CKD between the experimental group and the control group(P<0.01).The differences were significant(P<0.05).There were significant differences in Scr and eGFR between the control group and the control group(P<0.05).Before and after treatment,in BUN,there was significant difference between the control group and the control group(P<0.01).In Alb and 24h urine protein,there was no significant difference before and after treatment in t he control group(P>0.05);3.In AOPP,AGE and SOD,before the treatment,there was no significant difference between the two groups(P>0.05),which was comparable.And after treatment,between the two groups,it had significant differences in AOPP and AGE(P<0.05).In SOD,There was significant difference between the two groups(P<0.01);Between the experimental group and the control group,there were obvious differences in AOPP and AGE(P<0.05),and there was significant difference in SOD(P<0.01).There was no clear difference in AOPP,AGE and SOD between the two groups(P>0.05).4.In hs-CRP,IL-6 and TNF-a,before treatment,there was no significant difference between the two groups(P>0.05).Hs-CRP,IL-6 and TNF-a were significant in the two groups after treatment.The difference was significant(P<0.01).There were significant differences in hs-CRP,IL-6 and TNF-a between the experimental group and the control group(P<0.01).There was obvious difference in hs-CRP between the two groups(P<0.01),but IL-6 and TNF-a were no difference(P>0.05):5.For the safety indicators,in AST,ALT and K+,before treatment,there was no significant difference between the two groups(P>0.05).After treatment,there was no significant difference in those indicators between the two groups(P>0.05);Both of two groups,before and after treatment,there was no significant difference in the safety indicators(P>0.05).Conclusion1.The traditional Chinese medicine of invigorating the spleen and kidney and activating blood circulation decoction combined with the control of blood pressure,hypoglycemic and other basic treatments can effectively reduce the TCM syndrome scores of patients with the stage III-IV of CKD,improve the clinical symptoms of patients,and protect the renal function of patients with the stage ?-? of CKD;2.Invigorating the spleen and kidney and activating blood circulation decoction can effectively alleviate oxidative stress and micro-inflammatory state in patients with CKD ?-?;3.Invigorating the spleen and kidney and activating blood circulation decoction has good safety and can be safely applied to patients with CKD stage?-?.
