Objective Analyze the clinical characteristics of adult idiopathic thrombocytopenic purpura (idiopathic thrombocytopenic purpura ITP); evaluate the efficacy of differenttreatment methods; explore the prognostic factors related to ITP.Methods Collect the patients of ITP those who are older than 14 years ,diagnosed and treated from August 2002 to July 2009 in Affiliated Hospital of Ningxia Medical University, collate and analyze the relevant clinical data.Result 1. The percentage of the number of patients throughout the year were 31%, 25%, 25%, 19% respectively; statistical analysis showed that there are significant differences ,the gender in each age group were not identical; of 14-20 years old and > 60 years old ,the percentage of men and women is almost equivalent. of 21 -60-year-old patients ,female is the majority. There is 20 cases who have a clear infection of virus before sickness, accounting for 7.75% of total patients. 2. There is statistical difference in platelet count between the patients who have the symptoms of bleeding and not . Although the site of bleeding is different, but the count of platelet has no statistically significant. The patients who have skin, nose, gums and mouth, genital tract, gastrointestinal tract, eyes, urinary tract, brain hemorrhage accounted for 91%, 45%, 31%, 20%, 5%, 3%, 2% ,1% of the total number of bleeding patients. 3. The proportion of Marrow megakaryocytes increased, normal, reduced is 77%, 15%, 8%; the abnormal immunoglobulin and complement show thatC3, C4, lgM reduced and lgG, lgA elevated mostly; 25 cases of lymphocyte immune analysis, 19 patients are abnormal, abnormal ratio is 76%; 30 cases Helicobacter pylori was detected , 10 cases positive, the positive rate is 33%. 4. The recent efficient of glucocorticoid in First and subsequent visit patients, glucocorticoid and immunosuppressant, immunosuppressant alone and splenectomy is 85%, 90%, 93%, 73%, 97% respectively. 5. Compare 22 cases of refractory ITP patients with 29 patients having good response to the glucocorticoid. There is statistical significance in gender, cellular immunity, platelet peak after treatment, the speed of platelet recovery and the time of platelet stabilization(P <0.05). however there is no statistical significance in the number of bone marrow megakaryocyte, humoral immunity and the count of platelet before treatment(p> 0.05) .Conclusion 1. The incidence of Adult ITP is hidden and occurred in spring easily. Female patients are prevalent in 21-60 years old patients, and the gender is almost equivalent in younger than 20 years and older than 60 years. 2. Among ITP, the bleeding tendency has related to the palete level, however , there is no absolute relationship between the degree of blooding and platelet levels. So the risk of bleeding should be judged comprehensively in clinic. 3. Marrow megakaryocytes increased is the main type, the abnormalities of immunoglobulin and complement may be associated with the disease.4. Glucocorticoid is the first choice of treatment for adult ITP, and it also has a satisfactory effects on the return visits. Most patients who were treated by glucocorticoid, vincristine and splenectomy get good recent efficient generally. 5. Maybe the following factors can indicate better prognosis of ITP: Women,cellular immunity abnormality, the higher platelet peak, the faster platelet recovery and the longer platelet stability.
