Purpose Get though the comparsion between MRI image and arthroscopy, and research whether they have the relativity to refelt the serious degree of knee joint cartilage disease. That can direct the clinical doctors how to proceed an effective estimate and how to formulate a relevant treatment. In the meantime, can also be a effective index to estimate the treatment effect in the therapeutic process of traditional Chinese medicine.Method Each surface of knee cartilage is divided into 6 parts:surface of epicondylus medialis femoris, surface of epicondylus lateralis femoris, facies medialis of patella,facies lateralis of patella,facies medialis of tibial plateau,facies lateralis of tibial plateau. The doctor of performing the MRI make the diagnosis and the pathologic classification by MRI image before operation, on the basis of recht's grading standard. Write out the report. The doctor of performing arthroscopy make the classification by both the finding of arthroscopy and the image which collected by arthroscopy. Another one tabulate data contrasting the both reports.Result Total 240 joint sufaces of 40 knee joints. the arthroscopy and(or) MRI discoverrd 101 cartilage pathological changeses of different degree.Take the diagnosis of the arthroscopy as standard, evaluation all levels MRI diagnosis of accuracy, sensibility, specificity, consistency.The class 1:sensibility=58.62%, specificity=86.11%,the acreage below ROC curves is 0.852.Comparing to the opportunity line 0.5, the difference includes a statistical significance(P<0.0001).The class 2:sensibility=86.96%%, specificity=85.90%,the acreage below ROC curves is 0.870.Comparing to the opportunity line 0.5, the difference includes a statistical significance(P<0.0001).The class 3:sensibility=86.36%, specificity=96.20%,the acreage below ROC curves is 0.875.Comparing to the opportunity line 0.5, the difference includes a statistical significance(P<0.0001).The class 4:sensibility=94.44%, specificity=97.59%,the acreage below ROC curves is 0.940.Comparing to the opportunity line 0.5, the difference includes a statistical significance(P<0.0001).Conclusion Though the specificity of MRI to diagnose the knee OA joint cartilage disease is low. Taken together, that still remains high value in diagnosing the knee OA joint cartilage disease. And the cartilage disease more serious, the MRI diagnosis is more valuable. Moreover, before appearance of the joint cartilage injury, MRI can measure the change of cartilage matrix.appears appearance to learn to break use MRI and then can measure the variety of the cartilage radicle quality composition, can than earlier the joint mirror is one-step and discover cartilage earlier period pathological changes, earlier than using arthroscopy.
