Clinical Observation About Tao Hong Si Wu Tang Preventing DVT After Total Knee Arthroplasty

Objective:This paper discusses the clinical effects, the security and the mechanism of action of Tao Hong SiWuTang when it is applied in the preventive treatment of Deep Venous Thrombosis(DVT) caused by Total knee arthroplasty (TKA).Methods:Forty-one patients with osteoarthritis of knee from The First Affiliated Hospital of Nanjing University of Traditional Chinese Medicine have been selected as observation objects at random from November 2008 to March 2010 so as to carry on a comparative analysis of curative effect between treatment group and the control group. Forty-one patients who had received unilateral TKA are divided into two groups, one of which 21 patients (5 males plus 16 females), aged from 59 to 79 (the average age is 68.3) are under the treatment group, and take Tao Hong SiWuTang and Low Molecular Weight Heparin Calium (LMWH) after operation to protect DVT; the other group in which 20 patients (5 males plus 15 females), aged from 60 to 84 (the average age is 70.3)are under the control group of western medicine, and take LMWH to keep from DVT.Observation:To observe the podosoma VAS scores, the swelling degree of shanks, The variation of Platelet Count index, and to compare the haemorrhage and DVT situation after operation of the two groups.Results:By observing the swelling degree of shanks after surgery:there is no difference between the two groups three days after the operation; after 7 days, the curative effect of the treatment group of Chinese medicine is inferior to that of the control group of western medicine, thus the difference has statistical significance(P<0.05)seven days after the operation.According to the podosoma VAS scores after operation, the curative effect of the treatment group of Chinese medicine is prior to that of the control group of only LMWH, thus the difference has statistical significance(P<0.05).The amount of bleeding of the former one after the operation is inferior to that of the latter one, and the difference has statistical significance(P<0.05).According to the Platelet Count index, the difference has no statistical significance(P>0.05)three days after the operation. By the chi-square test, The rate of incidence of DVT of the former group after the operation is 4.76% while the rate is 10% of the latter one, and the difference has no statistical significance(P>0.05).Conclusion:Although the incidence of takeing Tao Hong SiWuTang and Low Molecular Weight Heparin Calium after the total knee arthroplasty to protect DVT had no significant effect with using LMWH only after TKA, but it can reduce the patients'postoperative pain and swell the degree of shanks after surgery, and it also can effectively reduce the the amount of bleeding of the former one after the operation without affecting the Platelet Count index.It can improve the quality of life in patients after TKA with a certain security.
