Significance And Expression Of Survivin And Caspase-3 In Hepatocellular Carcinoma

Objective: To investigate the expression of Survivin and Caspase-3 in the hepatocellular carcinoma (HCC) tissue and the pericarcinomatous liver tissue, approach the relationship betwee Survivin, Caspase-3 expression and parameters feature,as well as the correlation of Survivin and Caspase-3 in HCC. Methods: The expression of Survivin and Caspase-3 were detected with immunohistochemistry in 9 cases of normal liver tissue and 70 cases of HCC and pericarcinomatous tissue, while the expression of PCNA was detected with immunohistochemistry in 70 cases of HCC tisse. PCNA positive level was used to account Proliferation index(PCNA-PI), and cell apoptosis index(AI) was examinated with TUNEL technique. Then analyze the expression of Survivin and Caspase-3 combining the feature of clinic and pathology of patients. Results: (1) The expression of Survivin in HCC tissue is higher than those in pericarcinomatous tissue and normal liver tissue, while the expression of Caspasse-3 in HCC tissue is lower than those in pericarcinomatous tissue. the differences have statistical significance(P<0.05). (2) In HCC tissue, the average PCNA-PI of the Survivin positive group is higher than the negative(P<0.05), and the average AI of the Survivin positive group is lower than the negative(P<0.05). In HCC tissue, the average PCNA-PI of the Caspase-3 positive group is lower than the negative (P<0.05), and the average AI of the Caspase-3 positive group is higher than the negative(P<0.05). (3) There was negative correlation between the expression of Survivin and Caspase-3 in HCC(P<0.05 r=-0.355). (4) In HCC tissue, the expression of Survivin in cancer thrombosis group is higher than the no thrombosis, the tumor size >5cm group higher than the≤5cm, the histologic differentiation levelⅠ-Ⅱgroup lower than the levelⅢ-Ⅳ, and the stageⅠlower thanⅡ, stageⅡlowerⅢ, the differences have statistical significance(P<0.05). (5) In HCC tissue, the expression of Caspase-3 in HBsAg positive group is higher than the negitive,and the histologic differentiation levelⅠ-Ⅱgroup lower than the levelⅢ-Ⅳ, the differences have statistical significance(P<0.05). (6) The 70 patients average survival 25.00±1.73 months; the first,second,third year liverbility are 68.6%, 52.9%, 47.8%. Survival analyses show that the average surviva1 time of patients after surgery with the Survivn positive group was shorter than the negative, but the average surviva1 time of patients after surgery with the Caspase-3 positive group was longer than the negative, the differences have statistical significance(P<0.05). Conclusion: (1) Expression of Survivin and unexpression of Caspase-3 play an important role in the occurrence of HCC and the development process. (2) Survivin and Caspase-3 effect on the HCC cells proliferation and apoptosist. (3) Survivin and Caspase-3 may estimate the prognosis of HCC patients.
