Background and ObjectNasal polyp is one of the most frequently reported chronic diseases of nasal-nasal sinuses mucosa in clinic. Although many causative factors have been implicated in pathogenesis of nasal polyp, the exact etiology is still far from completely revealed. Most of scholars suggest that the Th1 and Th2 subgroups coexist in nasal polyps and the balance state of Th1/Th2 immune response is broken.Dendritic cells(DCs) are unique antigen-presenting cells with a ability to sensitize naive T lymphocytes and induce primary immune response. Human DCs can be divided into two distinct subsets(DC1 and DC2) which have characteristic markers on surface(CD11C and CD123).A great deal of work suggests that DC subclasses contribute significant polarizing influences on T helper cells differentiation. The Th cell could be differentiated into Th1 and Th2 by the effect of DCl and DC2 seperatively.We had found that there had DCs in nasal polyp tissues and the expression was higher than normal inferior turbinate mucosas. Therefore, we supposed that the DC subsets coexisted in nasal polyps and the state of them was imbalance. The aim of this study was to detect the expression of CD11C and CD123 in nasal polyps. We explored the role of DCl and DC2 in the pathogenesis of human nasal polyps.MethodsThe specimens were from 30 nasal polyp patients and 30 patients with deviation of nasal septum. All of the specimens were studied by hematoxylin and eosin(HE). Detected the S-100 for identifying CD11C+ DC. The experssion of CD11C+ DC and CD123+ DC in all the samples were detected by two-step immunohistochemistry staining. Tissue sections were observed through optical microscope. The appearance of brown-yellow pigmentation in cytoplasm was considerd as positive cell. Microscope was used to calculate positive cells and ascertain their expressive rate. The resulte experssed as norm士standard deviation. All the data were conducted by spss13.0, chisquare test and T test were used. The significance level was set at a P value of 0.05.Results(1) Nasal polyp tissue was morphologically characterized by extreme stromal oedema, gland hyperplasia, false multi-layer columnar ciliated epithelium with goblet cell proliferation. A lot of inflammatory cells infiltrating in nasal polyp tissues. 70% of the samples consisted of abundant T lymphocytes and plasma cells .(2) Both of the expression of CD11C+ DC and CD123+ DC in nasal polyp tissues were higher than that in control groups(P<0.01). The expression of CD11C+ DC compared with CD123+ DC in nasal polyps was no significant difference(P>0.05).(3) The expression of CD11C+ DC was significantly larger than CD123+ DC in nasal polyps which had positive cells(P<0.05).Conclusions(1) We find that there is a mixed DCl/DC2 pattern in nasal polyps, which indicates DCl and DC2 play an important role in the pathogenesis of nasal polyps. They may perform synergistic action as well as cooperation to take part in the whole process of nasal polyps.(2) DCl cells are predominant in nasal polyps may play an important role in promoting the infiltration of Th1 subgroup cells, which is the reason of imblance state of Th1/Th2 immune response. (3) Considerable lymphocytes and plasma cells infiltrate in 70% of nasal polyp samples. This type of histological may be have differences in region and race.It is worthy of attention.
