Objective:This study was by immunohistochemical staining to detect MIF、NF-κB p65and IL-1β expression in the tissue of nasal polyps and normal inferior turbinate, to analyze their relevance and explore their role in nasal polyps, and analyze the possible mechanism of action.Materials and Methods:Collection of35patients with nasal polyps who under FESS as the case group, and13patients who were performed septoplasty orthotics as the control group, we removal the polyp in the case group in the endoscopic surgery, while cutting part of tissue of inferior turbinate in the control group. The infiltrating results of EOS and others inflammatory cells through HE dyed, and the expression of MIF、NF-κB p65and IL-1β in tissue were measured by immunohistochemistry. To compare the positive expression of MIF、NF-κB p65and IL-1β between polyps group and control group, and analyze their relevance. VAS、Lund-Mackay、Lund-Kennedy were used to evaluate the degree of nasal polyps in patients, and correlation analysis was peformed between the disease severity and three factors of expression levels. All the expermental results using SPSS17.0software for the corresponding statistical analysis.Results:1. In nasal polyps group, HE dyed reveals epithelium is proliferous and thickening, the cilia is disorganized and lodging, part of pseudostratified ciliated columnar epithelium is replaced by the stratified squamous epithelium, interstitial edema obviously, and excessive gland tissue hyperplasia, and a large number of inflammatory cells infiltration, mainly eosinophils. The inferior turbinate mucosa tissue surface coating leveling ciliated columnar epithelium, the loose elastic fiber connective tissue within the tunica propria or interstitial,, rare inflammatory cell infiltration. The average of the EOS in nasal polyps group is higher than inferior turbinate mucosa tissue, the difference was statistically significant(P<0.05).2. In nasal polyps group, Positive expression of MIF mainly expressed in the epithelium, glands and stromal cell cytoplasm, for a tan Yellow dye. In the control group MIF is partly expressed in epithelium and glands in the cytoplasm, dyed with light, give priority to with weakly positive, expression level of MIF in nasal polyps is obviously higher than the inferior turbinate group (P <0.05). NF-κB p65positive expression in nasal polyps group throughout mucosa epithelial cells, inflammatory cells, glandular epithelial cells, endothelial cells and stromal cells or (and) in the cytoplasm and nucleus, its present tan, in the control group, with a little expression in mucosal epithelium and glandular epithelial cells and endothelial cells, theexpression level of NF-κB p65in nasal polyps group is significantly higher than the control group (P <0.05). The NF-κB p65nuclear activation rate is also significantly higher than the normal inferior turbinate mucosa (P <0.05). IL-1β expressed in nasal polyps on mucous membrane and cortex, plasma cells and inflammatory cells in the submucosa in the cytoplasm, tan grains, no expression in control group, IL-1β in nasal polyps is significantly higher than the control group(P<0.05).3. Spearman correlation analysis shows that MIF in nasal polyps group is positively correlated with the NF-κBp65(r=0.58, P <0.05), MIF is positively correlated with IL-1β(r=0.55, P <0.05), The NF-κB p65is positively correlated with IL-1β(r=0.49, P <0.05). There has significant positive correlation between NF-κB p65nuclear activation rate and MIF, IL-1β in nasal polyp tissues (r=0.61,0.56, P <0.05). MIF, IL-1β are positively correlated with EOS infiltration degree trend (r=0.44,0.51, P <0.05)4. Nasal polyps group of VAS score is7.21±1.58, nasal endoscopic examination score is7.34±3.62. Spearman correlation analysis shows VAS score is related to the level of MIF(r=0.67, P<0.05), VAS scores had no correlation with MIF level. Sinus CT, nasal endoscopic examination scores and the VAS scores as no correlation with NF-κB p65, NF-κB p65nuclear activation rate and IL-1β level. The nasal endoscopy scores with NF-κB p65nuclear activation rate and IL-1β stain positive significant correlation(r=0.71,0.69, P <0.05), and the nasal endoscopy scores had no correlation with NF-κB p65and IL-1βlevel(P>0.05).Conclusion:1. MIF, NF-κBp65and IL-1β in nasal polyps expression are significantly higher than normal inferior turbinate mucosa, and there are positive correlation between them, which prove that they may be through synergy promotes the development of the occurrence of nasal polyps, and there may exist in IL-1β-NF-κB-MIF approach in nasal polyps, this approach may be play an important role in the pathogenesis of nasal polyps, in the clinical treatment of nasal polyps we can block this way.2. MIF and NF-κBp65nuclear activation rate are significantly increased in nasal polyps and have a positive correlation with the severity of the nasal polyps, we can infer that MIF and NF-κBp65nuclear activation rate can be used as a nasal polyp severity and prognosis of a standard.3. The inferior turbinate mucosa in control group also has a small amount of NF-κB and MIF expression, it suggested that the NF-κB and MIF may participate in maintaining physiological function of inferior turbinate or with inferior turbinate the lightest sustained inflammation.
