| Objective:According to the theory of "Treat the internal diseaseoutside, both Internal and External" in Traditional Chinese Medicine (TCM), to observe the efficacy of treating herpes zoster with Ruyihuangjin Powder in TCM which is mixed with vinegar and combined with TDP through case-control study.Method:The subjects,78 in all, were the inpatients from the Dermatology Department of Wuhan 1st Hospital. They were divided into two groups,46 in the treatment and 32 in the control. The sex, age of onset, region, Erythema, blisters, pain and skin lesions area of grade course of the patients in both groups had no significant differences in statistics, and the clinical menifetations matched the herpes zoster described in "Clinical Dermatology", the 3rd editon, published by Phoenix Science Press. Patients of the both groups had got the same systemic therapy, which were treatments of normal antivirus (Ganciclovir Sodium for Injection 250mg ivdrip qd, Indometacin Tablets 25mg tid taken orally half an hour after meal) trophic nerve (Vitamin B1 and Cobamamide B12 Tablets both 1 pill po tid), and so on. The control group took only one TDP. everyday based on systemic therapy while the treatment group took Ruyijinhuang Powder mixed with vinegar (applied the Ruyijinhuang Powder which had been put on the gauze evenly to the damaged skin according to its size) as well as TDPbased on systemic, once per day,7 days as a course.After two weeks of observing on the two groups, the result was collected in lists. A comparison on the change of lesions score and TCM symptoms score in the first 3 days,5 days and the final end of the treatment was made between the two groups. Finally the Assessment of Efficacy was made according to "Linchuangjibing Zhengduanyiju Zhiyu Haozhuang Biaozhun".Results:1.78 cases of herpes zoster in patients aged 13-85 years, mean 59.96±15.57 years, the number of patients aged 50-70 years is 56 case, accounting for 71.79% Onset time is 2-10 days, an average of 5.76±2.21 days. The average number of treatment days:treatment group 6.70±1.82 days in control group 7.38±2.52 days. The number of days in the treatment of the two groups has no significant difference (t=1.381, p=0.171> 0.05).2. Total markedly effective rate were significantly different in the treatment group and control group(X2=4.064, P<0.05), whereas no significant difference in cure rate (X2=0.09, P> 0.05), the total efficiency is not significantly different (X2 =0.034, P>0.05). There are very significant differences in the efficacy of indices (t=3.452, p<0.01).3. The total score after treatment are very significant differences between treatment group and control group (t=-3.475, p<0.01) compared the grading of skin lesion symptoms after the treatment between two groups:erythema (t=-3.88, p<0.01) has a very significant difference, skin lesion area (t=-3.74, p<0.01) has a very significant difference. while blisters (t=0.34, p >0.05) and pain (t=-0.47, p> 0.05) no significant differences.4. The treatment group compared to the control group:Symptom score reduce index, erythema, blisters and pain have no substantial difference in the third day,while skin lesion area (t=2.17, P=0.03<0.05) has significant difference. Symptom score reduce index, erythema, pain and skin lesion area have no significant differences in the fifth day, while blisters has a very significant difference (t=-4.47, P<0.01)5. Between the efficacy of different courses of treatment group, no significant difference (X2=2.72, p=0.26). Between the treatment days of different courses, no significant difference (F=0.165, p=0.85)6. In both groups, opt-in patients after treatment did not show the phenomenon of exacerbations, One patient treated only the first day has a slight irritation, were not dealt with and adhere to treatment, the second treatment no longer respond, No complication other skin allergies, pain, ulceration and discomfort.Conclusions:1. The therapeutic effects of herpes zoster treated by vinegar concocting Jinhuang Powder with TDP compared with the control's is more effective.2. Judging from the improvement of skin lesions, the treatment group compared with the control group, more significant for erythema subsided, blisters collapse scar, repair of lesions, reducing lesion size.3. Juding from the relief of pain, Vinegar concocting Jinhuang Powder with TDP treat herpes zoster compared with the control's has no different significant.4. Juding from the short course, Vinegar concocting Jinhuang Powder with TDP treat herpes zoster compared with the control's has no different significant.5. Clinical application of the treatment of herpes zoster by vinegar concocting Jinhuang powder with TDP is safe, effective and tolerable side effects.
