The Psychopathological Effect Of Suicidal Event And Early Intervention To Witnesses On A Chinese College Sample

The aim of this study was to describe the prevenlence and symptoms of ASD, to examin the relationship between short-term responses and subsequent symptoms of posttraumatic stress disorder to a trauma, to explore the effect of intervention to the victims of the severe traumatic responses(ASD) in the witnesses of the traumatic event which a college student self-destructivly fell to the ground from the 7th-storey of the college building. The whole study is composed of three studies. 58 subjects of sample were recruited voluntarily from the witnesses of the suicidal event in a Chinese college. The quantitive research method was selected to use in the first two parts of this study, and the qualitative research method was used in the last part of this study. The research tools used in the study are here: the general condition scale about subjects and subjective feelings to the traumatic event, the impact of event scale(IES), symptoms checklist 90(scl-90), the simplified coping style questionnaire(SCSQ), The adolescent self-rating life events checklist(ASLEC), the social support rating scale(SSRS), the family assessment device(FAD), and the traumatic stress disorder clinical interview questionnaire.In the first part of this study, the validity and reliability of IES in the Chinese traumatic sample were testified and explored. The results showed that the internal consistency of IES was satisfaction, and total scale's Cronbach's a coefficient was 0.90; the intrusive factor's a was 0.88, the avoiding factor's a was 0.81; and that the test-retest stability of two weeks of IES was accepted, the total scale stability's coefficient r was 0.618; the intrusive factor's coefficient r was 0.634, the avoiding factor's coefficient r was 0.513. The results of this study also indicated that the twolist corelative coefficient(rb,) of criterion-related validity of IES was 0.67. The structure validity of IES was satisfaction too, and the result of factor analysis showed that IES might include the third factor indicating the dissociation after trauma beside the intrusive and avoiding two factors.In the second part of this study, the psychopathological effect of witnesses after the traumatic evevt were described, and the relative-factors of the psycho-effects were analysesed. The results of study showed that the prevalence of ASD in the sample was 0.69% two weeks after the event, and sex prevalence of ASD was nosignificant difference. The results of clinical interview indicated that 65.5% witnesses felt intens fear or horror, 51% witnesses.had peritraumatic dissociative symptoms, 84.5% witnesses had reexperiencing symptoms, 70.7% witnesses had avoiding symptoms, 53.4% witnesses had increased arousal symptoms, and 25.9% witnesses had clinical significantly distress impacting their some social function. After the event occurred, the peritraumatic dissociative symptoms appeared firstly(mean 1.12±1.81d), the increased arousal symptoms appeared secondly(2.55 ±2.67d), the reexperiencing symptoms appeared thirdly(mean3.59±2.99d ), and the avoiding symptoms appeared lastly(mean 4.85±2.92d). After two weeks of the event, the witnesses of severe stress reaction(IES>20) had lower mental health level than the witnesses of mild stress reaction(IES<20), and the former compared to the later had much used "the negative coping style" , felt much stress to the traumatic event, had experienced much effects of life events one year before the traumatic event . In the 5th week after the event , the witnesses who had peritraumatic dissociative symptoms early-term after the traumatic event showed significantly severer symptoms of IES intrusion and avoidance, and showed significantly severer each cluster symptom of PTSD compared to the witnesses who had no peritraumatic dissociative symptoms early-term (p<0.01-0.05).
