Individualized education is an important principle of special education, and Individualized Educational Program (IEP) is a basic instrument to carry out individualized education and to ensure that special children can receive high quality education. IEP forms are tools to document educational programs for special children. They are the foundation to develop and implement IEP. IEP has been studied for over 30 years in some countries. In these countries, IEP function and supervision system have been set up. In our country, there are still different opinions about the functions and significance of IEP, so it is very necessary to do some research work on IEP. This dissertation is to development an appropriate IEP form for regular teachers to use, and to help special children in regular classes receive appropriate education. This dissertation will also discuss the contents of the IEP form.This dissertation can be divided into four sections. In the first section, the writer introduced IEP's concept and connotation, origin and development, then discuss the main problems in IEP. In the second section, the writer introduced the IEPs' contents, formats and requests in the U.S.A, Taiwan and Shanghai, then made some suggestions about the development of the IEP form for special children in regular classes. In the third section, the writer investigated the conditions of IEPs in Shanghai, including the condition of IEP forms, and the condition of IEP development and implementation .Then the writer analyzed the predominance and shortcomings of Shanghai's IEPs. In the end of this part, the writer pointed out the issues we should pay attention to when we develop the IEP form in our country. In the last section, the writer introduced the main assumptions in the process of IEP development, the contents of the origin form. Then the writer let regular teachers try out the form. During this process, the writer modified the form for three times according to teacher's feedback and other articles' suggestions. The writer also investigated the quality of the form.According to the research findings, the IEP form for special children in regular classes should include following contents: students' basic information introduction, family basic information introduction, assessment of the students' capabilities, teaching measures, annual goals (including benchmarks or short-term objectives) and transition services. IEP should fulfill in the ways of ticking off and opening writing. According to the IEP quality investigation, this form showed much predominance compared with other forms in Shanghai.
