Objective:Pleural effusion is a common and frequently-occurring disease in clinic. Although the research of western medicine in the diagnosis and treatment of pleural effusion continue to make progress, with the aging population and the changes of environment and lifestyle, there are still many problems must be faced and solved. Such as patients of the pleural effusion due to tuberculosis pleurisy, whose effusion has been absorbed after some kind of treatments, still feel chest pain because of the pleural adhesions or pleural reaction; the treatment of parapneumonic effusions by using antibiotics is not always useful. Pleural effusion belongs to "Xuan Yin" in traditional Chinese medicine. Zhang Zhongjing put forward the disease of "Xuan Yin" and treated it with "Shi zao Decoction". Traditional Chinese medicine has accumulated rich experience in the etiology, pathogenesis, treatment of "Xuan Yin". It can complement with western medicine to solve clinical problems. Professor Wang Xuejing, who learned from the second session of The Chinese Medicine Master Professor Chao Enxiang, has accumulated rich experience in the treatment of pleural effusion during more than thirty years of clinic work. The purpose of this paper is to summarize Professor Wang xuejing’s theory of traditional Chinese medicine and treatment experience on pleural effusion and to provide a new thought for Chinese medicine treatment of this disease. Methods:Through studying the cases of pleural effusion which Professor Wang xuejing had treated, to summarize Professor Wang’s experience of this disease. Conclusion:Through summarize Professor Wang’s clinic experience of treating pleural effusion, Professor Wang thinks we should combine syndrome differentiation with disease differentiation during the treatment of pleural effusion. Syndrome differentiation is to find out what kind of syndrome the patient has. Besides to find out the disease in Traditional Chinese Medicine, the disease differentiation emphasizes to find out what kind of disease that causes pleural effusion. Because through the clinic research we find out that pleural effusion causes by different diseases, its syndrome differentiation will be different. So to find out the cause of pleural effusion is helpful to the Traditional Chinese Medicine. Professor Wang believes there are three classes of syndrome of pleural effusion in clinic:collaterals stasis syndrome, deficiency of both qi and blood syndrome, deficiency of yang and edema syndrome. We can treat the patients who have collaterals stasis syndrome with fu yuan huo xue tang, treat the one who have deficiency of both qi and blood syndrome with bazhentang and yang he tang, and treat the patients who have deficiency of yang and edema syndrome with ling gui zhu gan tang or zhen wu tang. For the one who has deficiency of both qi and blood syndrome, especially causes by chronic empyema, Professor Wang believes therapy of promoting growth of tissue regeneration, removing slough is important. For the one who has enough antipathogenic Qi, he can remove slough and promote growth of tissue regeneration, but for the one who has deficiency of both qi and blood syndrome, it should first promote growth of tissue regeneration and then remove slough, otherwise it will become remove slough and promote slough. If only our body have enough antipathogenic Qi, it can use Qi and blood to promote growth of tissue regeneration. If deficiency of both Qi and blood, it can not remove slough and promote growth of tissue regeneration. Thus for the one who have deficiency of both qi and blood syndrome, is very important to tonify qi and blood. Qi and blood sufficient can remove slough and promote growth of tissue regeneration. It provides a new thought for Chinese medicine in the treatment of pleural effusion.
