| BackgroundNon-small cell lung cancer is one of the most common malignant tumor, its incidence and mortality in our country is the top of all the malignant tumors. Among patients with lung cancer in our country, the vast majority of patients with non-small cell lung cancer at the time of diagnosis has shift and lost the surgery opportunity. However,with the continued exploration of treatment methods and techniques, the treating selection of treatment of non-small cell lung cancer increases. At present western medicine for thetreatment of advanced non-small cell lung cancer includes traditional medical treatments such as palliative chemotherapy, radiation therapy, nutritional support and new ways such as targeted therapy, immune therapy, and minimally invasive treatment as well. Various treatment has certain effectiveness, but each treatment methods have its corresponding limitations.Playing TCM treatment features, combine Chinese with western medicine treatment may get an effective treatment methods.through researchingthe metabolism of Erchen decoction (ECD) inhibition metastasis to non-small cell lungcancer for many years and early clinical trial, My mentor found that ECD has certain inhibitory effect in patients who are non-small cell lung cancer which has metastasises with phlegm syndrome. and can not only control the development of tumors and metastases from the primary lesion, but also improve the general condition, quality of life, reduce the side effect of chemotherapy.Research purposeThrough controlled clinical research, observe curative effects include clinical symptom, the improvement of the quality of life,immune level and tumor marker applicating ECD for treatment of phlegm syndrome in patients with non-small cell lung cancer metastasis physical situation,compare curative effect difference with pure chemotherapy group at the same time, the more clear the role of the treatment of traditional Chinese medicine in the western medicine treatment, so as to better guide clinical treatment.Research methodsAdopting the method of perspective randomized, controlled observational study,To collect metastasis of non-small-cell lung cancer NSCLC patients undergoing systemic chemotherapy who meet the inclusion criteria during January 2015 to December 2015, by the Chinese academy of medical sciences cancer hospital, Beijing university of Chinese medicine, the third affiliated hospital oncology,the patients were divided into two group, both groups were treated by chemotherapy with platinum drugs on basic,the experimental group were given ECD.With a treatment course of three weeks in both groups, the therapeutic effect was analyzed by observation of KPS score,lung cancer clinical symptom score and quality of life score, immune function, tumor marker.Using SPSS21.0 statistical analysis software for statistical analysis.[Research results]1. The curative effect of general physical condition ascites:the Karnofsky of the experimental group increases after treatment the difference was statistically significant (P< 0.05).the control group after treatment scores rise,There was no significant difference between two groups (P> 0.05), suggesting that after Platinum-containing chemotherapy combined with ECG treatment can improve physical condition in patients with NSCLC.2.To improve symptom integral:The effective rate of the experimental group and control group is 67% and 27% respectively, the difference is statistically significant (P< 0.05).Comparison of the two groups’ symptom scores,the experimental group is superior to control group, the difference is statistically significant (P<0.05).The experimental group improved in cough, sputum, fatigue symptoms the difference is statistically significant (P< 0.05),while there is not statistically significant in the control one (P> 0.05).3. In terms of quality of life,Patients was investigated by filling out QLQ-C30, QLQ-LC13 life questionnaire.The result shows that the overall health level of the experimental group is increased (P< 0.05),while the control one is decreased after treatment(P>0.05). there is no statistically significant difference compared with the control group (P>0.05).To functional scales, the experimental group has improved significantly in terms of physical function, social function, statistically significant difference (P<0.05); The control group in physical function, emotional function improved significantly, statistically significant difference (P< 0.05); there is statistically significant difference in social function compared with the control group (P<0.05).To symptom Scales, Symptoms child volume table in the itself treatment before and after compared:experiment group in respiratory symptoms aspects such as breathing difficulty,shortness of breathing, cough, hemoptysis etc, and other symptoms aspects such as tired and insomnia and hair loss has significantly improved, and in pain aspects has must improved, differences has statistics meaning (P<0.05); the control group in respiratory symptoms aspects has must of improved; there is not statistically significant in the control one (P> 0.05).4. Immune index:the activity of CD4+cells are increased significantly and the test group is superior to the control one(P<0.05)5.Tumor marker:After treatment, the level of NSE of the experimental group is decreased (P<0.05),but there is no statistically significant difference compare between groups (P>0.05).Research conclusions① ECD combined with chemotherapy can improve part of NSCLC patients’ physical conditions to some extent.② ECD combined with chemotherapy is more advantageous to improve part of the lung cancer patients with clinical symptoms.③ ECD combined with chemotherapy can help patients with their quality of life to improve④ ECD combined with chemotherapy can probable reduce the immune injury cause d by chemotherapy or immunosu-ppression and play a role of immune injury repair as well.combined with chemotherapy may have effects on suppressing tumor marker NSE, But ECD on lung tumor control needs to be the subject of further study. |
