Investigation On The Status Quo Of Hand Health Of ICU Medical Personnel And Its Influencing Factors

ICU is the important department to prevent HAIs, improving hand hygiene compliance of ICU healthcare workers is the effective measures to decrease HAIs. From studies of home and abroad, bacteria is rich in hands of ICU healthcare workers, hand hygiene compliance is variable. Only to learn the key factors of affecting hand hygiene compliance of ICU healthcare workers can sets up effective measures, then to improve the condition of ICU healthcare workers’ hand hygiene. So discussion about current situation and factors of ICU healthcare workers’ hand hygiene is necessary.Research purposes:(1) Using the measures of Microbiological Methods to survey ICU healthcare workers’ hand hygiene. (2) Surveying the factors that affecting healthcare workers’ hand hygiene.Research method:This is descriptive study, which use the convenience sample way to extract bacteria samples from the ICU healthcare workers’hands. Under the guidance of KAP theory and the theory of planned behavior, we give "Hand Hygiene Knowledge Questionnaire for Health-Care Workers" and "Perception Survey for Health-Care Workers" to ICU healthcare workers, we use "Hand Hygiene Knowledge Questionnaire for Health-Care Workers" to investigate healthcare workers’knowledge, and use "Perception Survey for Health-Care Workers" to investigate hand hygiene attitude (behavior attitude), social pressures influence (subjective norm), the difficulty degree to perform hand hygiene (perceived behavioral control), self-assessment of hand hygiene compliance and attitude of hand hygiene promotion measures. Then, we analyze the factors affecting the healthcare workers hand hygiene compliance comprehensively.Results:(1) The colonie number of ICU healthcare workers hand was 34.34±56.97, there are significant differences between different jobs and different hospitals. Further analysis the bacteria, we found Staphylococcus aureus in the two hospital. Through bacterial resistance testing, we found HLGR in the hands of the B hospital healthcare workers. And H hospital ICU healthcare workers’ hand checked out three kinds of drug-resistant bacteria, MDRP, MDRA and CRE. MDRP has the highest detection rate. According to the standard of hand hygiene, the pass rate of ICU healthcare workers hand hygiene is 74.8%, there are no significant differences between nurses and docters. (2) The hand hygiene knowledge score of ICU healthcare workers is 16.45±2.34. In the questionnaire of perception survey in hand hygiene, the hand hygiene attitude score of ICU healthcare workers is 10.59±1.46, the score of hand hygiene promote measures effectiveness is 50.87±5.46, the score of social pressures on the ICU healthcare workers hand hygiene is 19.20±2.54, the score of difficulty degree to perform hand hygiene is 4.13±2.14. Using the three factors as the dependent variable to analyze the relationship between hand hygiene compliance, we found that ICU healthcare workers hand hygiene compliance mainly affected by the subjective norm and perceived behavioral control. The greater influence of social pressure on healthcare workers, the hand hygiene more easier, then the hand hygiene compliance will be better.Conclusion:ICU healthcare workers hand hygiene pass rate is good, but the amount of bacterial is different between different jobs. Through the microbiological analyzing of ICU healthcare workers bacteria, we found that Staphylococcus aureus is more widespread, should strengthen the relevant protection. ICU healthcare workers hand checked out four kinds of resistant strains. The result shows that the spread of drug-resistant strains exist in the ICU, this increase the risk of patients get hospital infection greatly. ICU hospital infection management should focus on strengthening protection. The ICU healthcare workers hand hygiene knowledge is generally good, but the doctor hand hygiene knowledge is worse than nurses. In this case, we should strengthen the ICU doctors hand hygiene knowledge education. For ICU medical staff hand hygiene compliance related factors were analyzed, ICU healthcare workers hand hygiene mainly affected bt their subjective norms and perceived behavioral control. We recommend further measures for the relevant factors, strengthening the education of healthcare workers hand hygiene knowledge, appealing leadership, patients and their families to participate, improving hand hygiene facilities and other measures. Using these measure to improve ICU healthcare workers hand hygiene compliance, further to improve the healthcare workers hand hygiene condition, and reduce the risk of the ICU hospital infection.
