The study is aimed at designing the Test Self-efficacy Questionnaire specializing in the measurement of students’ self-efficacy on tests. Guided by the general procedure of questionnaire compilation, the study adopted both a top-down perspective to systematically review the literature on perceived academic self-efficacy and a bottom-up perspective to conduct case interviews with third-year junior high students in an attempt to conclude the experience on test anxiety counseling. Meanwhile, based on the research outcomes of the former students on this topic and upon integration and improvement of the studies in this respect, the study winds up with the Handbook of Group Counseling on Test Self-efficacy, a professional and specialized reference to test self-efficacy interventions which is proved to be instructive in intervention practices.Participants with low self-efficacy and high test anxiety were screened out by the self-designed Test Self-efficacy Questionnaire with tested reliability and validity and Zheng Richang’s Test Anxiety Scale. They were then put into an experiment group and a control group, both of whom went through pretest and posttest as the general mode of experiment to test the validity of the Handbook of Group Counseling on Test Self-efficacy.The main outcomes of the study are as follows: firstly, the self-designed Test Self-efficacy Questionnaire, through internal consistency reliability test, split-half reliability test, exploratory factor analysis, confirmatory factor analysis as well as tests on other related statistical indicators, turns out to be a well-structured 22-itemed questionnaire highly reliable and effective in revealing the test self-efficacy of third-year junior high students; secondly, students get higher scores in their posttest with reduced anxiety and marked elevation of self-efficacy after the intervention under the guidance of the Handbook of Group Counseling on Test Self-efficacy, which convincingly proves the validity of the structure and the content of this book. Hopefully, it would be of some help to teachers and students in the graduating year of junior high school.
