Objective To study the age distribution and the ageing status of the medical insurance population, and their demand for medical services among different ages. Revealed the distribution of medical expenses, and the expenditures of medical insurance fund. Assess the medical insurance fund balance by the impact of the aging population.Method Several representative areas were selected for investigations and studies: Tianjin, Hunan, Fujian, Changsha, Qinhuangdao, Suzhou, Yancheng .Collect the data of social medical insurance system and policies, the annual payments, the number of participants, and pay expenditures in the region (for regional), mainly from the medical insurance management center.Result In the investigation areas, growth in the number of participants is the largest in Tianjin, increased from 1,396,400 people to 3,442,000 people; Tianjin's average growth rate is also the fastest, reaching 29.30 percent, But after 2004 the annual growth rate of all regions in gradually declining. In 2001, 22.50% participants in Tianjin were over 60 years, 1.90 times of that in Yancheng (11.87%). In 2006, Yancheng has already reached 14.30%, and other regions were more than 16.00%, three areas (Hunan straight 21.93%, 22.86%, Tianjin, Changsha 21.37%) were more than 20.00%. Especially the age proportion over 65 years has grown rapidly, the highest was Hunan, has reached 32.54 percentage points. The expenditure of the over-60-year age group was 2 to 6 times of the age group under 60 years generally, Similarly, the over-65-year age group was 2 to 7 times of the under-65-years age group. the medical expenses proportion of over-60-years persistently increased. There are two positive factors (the preserving and increasing the coverage) and three negative factors (age, expenses, raise) impact the medical insurance fund balances.Conclusion (1) the number of participants has increased year by year, but the trend of increase rate was gradually declining. (2) The ageing statuses of the population were imbalance between regions, and the aging trend were more obvious than the general population. The proportion of the over-60-years population growth soon, and the average age is improving, and medical insurance among the elderly population has a high starting point, to reach a higher level and the proportion of old participants increases even faster(3) Urban elderly population will increase the basic medical insurance payment expenditure, and the elderly population pays no medical insurance, so that medical insurance fund deficits and the existence of the gradual increase in risk.(4)The age factor gives a negative impact to the medical insurance fund balance. This shows that the aging impact on the stable operation of medical insurance seriously.
