The Clinical And Experimental Research Of Qing Dan Guan Chang Fang On Chronic Renal Failure

Abstract/Summary:
Objective:To observe Xiezhuo Jiedu and Wenyang Huoxue enema treatment on patients with chronic renal failure (CRF), assess the effects of this treatment on renal function and pathology of 5/6 nephrectomy rats with CRF and explore the mechanism.Method:1. Clinical reseach:100 patients with CRF were randomly divided into experimental and control groups. The experimental group were treated with Qing Dan Guan Chang Fang besides conventional treatment, once a day, while the control group treated with ai xi te,30 pills once. After 3 months' treatment, we observed the changes in symptom score of renal function, hemoglobin (Hb), parathyroid hormone (PTH) and TCM Hou of spleen deficiency and turbid stasis block, comparing the different clinical effects of the two treatments.2. Animal experiments:10 of 60 male Wistar rats were randomly divided into normal group and the rest were established as CRF models by 5/6 nephrectomy. A week after operation, models that were established as expected testing by blood taken from the tail vein were randomly divided into 4 groups:model group, ai xi te group (control group), the Qing Dan Guan Chang Fang low-dose group (low-dose group), and the Qing Dan Guan Chang Fang high-dose group (high-dose group). Total five groups were then given different concentration and dose of drugs and we observed changes in renal function, PTH, Ca2+, P3+, red blood cell (RBC), Hb, HCT and renal pathology before and after treatment. Results:1. Clinical reseach:After three months' treatment, compared with the control group, the creatinine (Scr), blood urea nitrogen (Bun) and uric acid (UA) of patients from experimental group improved significantly (P<0.01); the laboratory indicators of PTH, Ca2+ and P3+ improved significantly (P<0.01); and the hemoglobin increased (P<0.05).2. Animal experiments:Qing Dan Guan Chang Fang can ignificantly delay 5/6 nephrectomy rats the growth trend of Scrand Bun (P<0.01). After 12 week's treatment, there is a significantly difference in the growth trend of PTH and P3+, and in the decreasing trend of Ca2+ among high-dose group, model group and control group. At the same time, low-dose group and high-dose group can delay anemia increasing, comparing with control group (P<0.01) and model group (P<0.05). Compared with control group and model group, rats from two groups treated with TCM have ruddier kidney and lighter pathological lesions. Conclusion:Clinical researches have shown that Qing Dan Guan Chang Fang can improve the renal function, correct Ca2+ and P3+ disorder, reduce PTH and improve anemia. While animal experiments have shown that Qing nitrogen enema can protect the renal function of 5/6 nephrectomy rats and reduce the renal pathological damage. The mechanism may be related to reducing the residual renal glomerular sclerosis and reducing the damage of residual renal interstitial-tubular tissue by blood circulation and Xiezhuo, resulted in delay of the progress of renal failure.
