Yiqihuoxue Qingre Wet Treatment Of Chronic Nephritis Clinical And Experimental Research

The method of replenish Qi and activate blood, clear up heat and drain dampnesscirculation (益气活血,清热利湿法) is summaried by DAI XiWen professor to treat chronic glomerulonephritis with Qi deficiency blood stagnation accompanied by dampness heat (气虚血瘀兼湿热证) form long-term clinic practice. Clinical and experimental study verified the clinical effect to treat chronic glomerulonephritis and experimental study investigated the renoprotective effect and mechanism of replenish Qi and clear heat extract (益气清热膏) in adriamycin nephritic rats. Clinical study: 30 patients with chronic glomerulonephritis and Qi deficiency blood stagnation accompanied by dampness heat were mainly treated by the method of replenish Qi and activate blood, clear up heat and drain dampnesscirculation. The symptomatic scores and main laboratory markers were compared before treatment and after 3 months treatment. The improvement rate is 90.0% and the remission rate is 23.3% after treatment. Compared with before treatment and after treatment, the total score of symptom is obviously decreased(P<0.001),urine protein and urinary sediment RBC are decreased, serum total protein, albumin and immune globulin(IgA,IgG) are increased and total cholesterol is decreased. Experimental study: Not only normal rats but also adriamycin nephritic rats were treated with saline, replenish Qi and clear heat extract and fosinopril sodium(MONOPRIL) in 2rd week after operation. Urinary protein excretion, albumin, total cholesterol ,LDL, HDL, serum creatinine and BUN were measured before treatment and after 6 weeks treatment. Measangial cell index and measangial matrix index were assessed after 6 weeks treatment. Moreover immunohistochemical stain for type Ⅳ collagen, fibronectin and laminin were performed after treatment. Proteinuria of replenish Qi and clear heat extract treated rats was significantly reduced and plasm albumin was obviously improved after 6 weeks treatment compared with those before treatment. The proliferation of mesangial cells and the synthesis of mesangial matrix were inhibited in the replenish Qi and clear heat extract treated rats. Similar results were achieved in the fosinopril sodium treated rats.Conclusion: The method of of replenish Qi and activate blood and drain dampnesscirculation has better effect in .paint with chronic glomerulonephritis and replenish Qi and clear heat extract has a renoprotective effect in adriamycin nephrosis rats. Similar results were achieved in the fosinopril sodium treated rats. The mechanism maybe is the proliferation of mesangial cells and the synthesis of mesangial matrix are inhibited.
