Objective To observe the effect of Shen Di Granule on serum Col I,Col III,FN and Megsin in patients with chronic glomerulonephritis.Methods:From August 2016 to December-2017,60 CGN patients who met the inclusion criteria were selected and randomly divided into the treatment group and the control group,30 cases in each group,and 20 cases in the normal group.The treatment group was treated with ginseng granules 10 g each time,3 times per day dosed by warm water;control group was given valsartan orally,80 mg each time,1 times per day for 12 weeks.Observed two groups of patients before and after treatment for 24 h urine protein quantitative(24Hupr),blood urea nitrogen(BUN),serum creatinine(Scr),estimated glomerular filtration rate(eGFR),serum Col ?,Col ?,FN,Megsin level changes.Results1.In the treatment process,3 cases were eliminated In the treatment group and 3 cases were eliminated in the control group because of the factors such as the patient's own cause and the progress of the disease.The final treatment group,the control group 27 cases,the normal group 20 cases entered the statistical analysis.2.24 H urine protein quantitative: Respectively before and after treatment 4,8,12 weeks to preserve CGN patients 24 h urine samples,the results show that the treatment group and control group in 24 hupr before and 4 weeks after treatment was no significant difference(P>0.05);In the treatment group and control group,24 HUPr was significantly reduced after 8 weeks of treatment,and there were significant differences(P<0.05).3.Serum collagen type-?and collagen type-?:Before the treatment,control group and treatment group compared with normal group,Col?,Col ? levels are higher,there are significant differences(P<0.05);After treatment,the control group and treatment group Col?,Col ? levels drop,compared with before treatment with significantdifference(P<0.05);After treatment,the treatment group compared with control group,the Col ?,Col ? falling level more clearly,there are significant differences(P < 0.05).4.Serum fibronectin(FN):Before treatment,the control group and the treatment group were compared with the normal group,and the FN level increased,with significant difference(P<0.05).After treatment,the level of FN in the control group and the treatment group decreased,which was significantly different from that before treatment(P<0.05).After treatment,the decrease of FN was more obvious in the treatment group compared with the control group,with significant difference(P<0.05).5.Serum Megsin:Before treatment,the control group and the treatment group were compared with the normal group,and the Megsin level was increased,with significant difference(P<0.05).After treatment,the level of Megsin in the control group and the treatment group decreased,which was significantly different from that before treatment(P<0.05).After treatment,the decrease of Megsin was more obvious in the treatment group compared with the control group,with significant difference(P<0.05).6.Comparison of renal function before and after treatment: before treatment,there was no significant difference in serum creatinine,urea nitrogen,glomerular filtration rate compared with post-treatment blood creatinine,urea nitrogen and glomerular filtration rate(P<0.05).7.Comparison of blood pressure before and after treatment:The treatment group have 27 research object,and about 14 people blood pressure exceed 140/90 mmHg.The control group have 27 research object and 15 have Hypertension.All of the patients were normal after treatment(below 140/90mmHg).8.There was no significant change in blood image,liver function,electrocardiogram in patients taking Shen Di granule compared before treatment.Conclusion1.Chronic glomerulonephritis patients extracellular matrix components such as collagen type-?,collagen type-?,protein fiber connection and megsin significantly increased,its progressive accumulation exacerbate glomerular sclerosis,lead to glomerulardamage increase.2.Shen Di granule can reduce the extracellular matrix components such as collagen type-?,collagen type-?,fibronectin and megsin,and then reduce the damage of kidney.3.In the whole process of experimental study,the patients with chronic nephritis with oral Shen Di granules have no adverse reaction.It shows that the patients are safe.Objective Establish the model of mesangial proliferative glomerulonephritis rats,the detection of each group rats serum and kidney tissue Col ?,Col ?,FN,Megsin expression level,24 H urine proteinr,kidney pathological condition,to explore the effect of Shen Di granules on Ms PGN rat extracellular matrix Col I,Col III,FN and Megsin.Methods60 male SD rats,with a weight of 180-220 g,were fed in the animal room of the experimental building of Anhui University of traditional Chinese medicine for 1 weeks.According to random sampling,10 rats were randomly selected as the normal group.The remaining rats were made Ms PGN models.They were randomly divided into 3groups: model group,control group(valsartan group),and experimental group(Shen Di granule group).The rats in the model after completion of second day of administration,the experimental group treated ginseng particles in aqueous solution by gavage(dose:0.8g/200g-D);control group received Valsartan Capsules configuration into solution by gavage(dose: 1.03mg/100g-D);model group and normal group were given intragastric administration,warm water(3ml)all 12 weeks end.After anesthesia,rats were anesthetized.Blood and kidney tissues were taken to detect blood urea nitrogen(BUN),serum creatinine(Scr),24 H urinary protein quantitation,Col I,Col III,FN and Megsin levels in serum and kidney tissues,and pathological changes in renal tissues were observed.Result1.General situation of rats: the normal group rats are sensitive,free of movement,and glossy and glossy.After the successful model,the rats appeared to be depressed,prone to sleep,diet and water decrease,and the hair was dark and without any color.Continuous dosing lavage after 12 weeks,the experimental group and control group rats better mental state,hair luster,activity,diet,increase water intake,weight gain,better situation than the control group and experimental group rats obviously.The rats in the model group showed no significant improvement.2.24 H urine protein quantitation: Before the treatment model group,the control group and the experimental group,24 HUPr increased,which was significantly different from the normal group(P<0.05).There was no significant difference in urinary protein after treatment between normal group and model group(P<0.05).After 6 weeks of treatment,the urine protein of rats in the experimental group and the control group decreased,which was significantly different from that in the model group(P<0.05).After 12 weeks,24 HUPr in the experimental group was significantly lower than the model group,with significant difference(P<0.05).3.Comparison of Scr and BUN: there was statistically significant difference between the increase of rat BUN and Scr in the model group and the normal group(P<0.05).After treatment,both the control group and the treatment group BUN and Scr decreased,which was statistically significant compared with the model group(P<0.05).There was no statistically significant difference in BUN and Scr in the experimental group and control group(P>0.05).4.Serum collagen type-?and collagen type-?: model group rats serum and kidney tissue of Col I and Col III was higher than that in normal group,the difference was statistically significant(P<0.05);the experimental group and the control group in serum and kidney tissue of Col I and Col III were lower than the model group,there was statistically significant difference(P<0.05);experimental group kidney tissue and Col I and Col III is lower than the control group,the difference was statistically significant(P<0.05).5.Serum FN: The serum FN in the model group was higher than the normal group,with significant difference(P<0.05).After treatment,the serum FN in the control group and the experimental group decreased,and the comparison with the model group was significantly different(P<0.05).There was significant difference between the experimental group and the control group(P<0.05).6.Serum Megsin: The serum Megsin of the model group was higher than the normal group,with significant difference(P<0.05);After treatment,the serum Megsin in the control group and the experimental group decreased,and there was significant difference with the model group(P<0.05).There was significant difference between the experimental group and the control group(P<0.05).7.Renal pathology: glomerular mesangial area shows diffuse hyperplasia of mesangial cells and mesangial matrix,and sometimes glomerular segmental sclerosis is seen.The experimental group and the control group: there were mild mesangial cells in glomeruli of two groups.The mesangial matrix hyperplasia and basement membrane thickness of Shen Di granule group were lighter than those of valsartan group,indicating that ginseng granule could inhibit the proliferation of mesangial cells in Ms PGN rats.Conclusion1.The Collagen type-?,Collagen type-?,Fibronectin and Megsin of the mesangial region and serum of the Ms PGN rats increased significantly,and the progressive accumulation of the matrix could aggravate the glomerular damage.2.Shen Di granule can reduce the extracellular matrix of Ms PGN rats,such as collagen type-?,collagen type-?,fibronectin,cut Megsin level,and then reduce the urine protein,reduce the renal pathological damage,protect the kidney.
