Objective: To study the relation between the expression of neural cell adhesion molecule (NCAM) ,DPC4 and perineural invasion in patients with bile duct cancer.To explore the role of NCAM and DPC4 which play in the perineural invasion of bile duct cancer and its possible mechanism. Thus provide a valuable referring marker used to judge the tendency of perineural invasion of carcinoma of bile duct and its prognosis.Method: NCAM and DPC4 expression was studied by immunohistochemical staining(S-P method) in 52 archived specimens of histologically proven bile duct cancer and in 16 cases of normal bile duct tissues.We observed and analysed: (1) the relationship between the expression of NCAM,DPC4 and perineural invasion of bile duct cancer. (2) the association of NCAM ,DPC4 and various pathological type, the degree of tissue differenciation ,localization of tumor etc.Result: The expression of NCAM and DPC4 was significant differences among different histopathological types and degrees of differentation of bile duct cancer (PO.01). DPC4 low expression tumours were associated with a higher incidence of perineural invasion than DPC4 normal expression (PO.01). The perineural invasion rate of tumours stained positively for NCAM was higher than in those with negative NCAM staining (P<0.05). The expression of NCAM and DPC4 in carcinoma of bile duct of various pathological type indicated that the expression of NCAM was increased with the enhancemat of the malignant degree of carcinoma,while the expression of DPC4 was decreased. There seemed to be no correlation between expression ofNCAM ,DPC4 in carcinma of bile duct and patients' age, gender, the size of tumor and localization of tumor.Condusion: It was suggested that DPC4 low-expression and NCAM overexpression might play significant role in perineural invasion in bile duct cancer. NCAM may be an important biological marker used to judge the prognosis and malignant degree of bile duct cancer and predict its tendency of perineural invasion.
