Congestive heart failure (CHF) is a great problem of public health. It is reported that the death rate of CHF is much higher than that of cardia-angiopathy disease at the same time. So, it is our responsibility to find more effective therapies for CHF . Modern therapy for CHF has transformed from a short-term improvement hemodynamics to a long-term prosthetic strategy that alters the biological nature of failing heart. Redressing abnormality of hemodynamics will ameliorate the patient's clinical symptoms, though it is still an important therapy.A lot of researches have been made on the effect of Shengmai injection on CHF patients, but most of them are on its effect after a period of course treatment. The research on the prompt effect of Shengmai injection on the homodynamic changes is rarely known. The paper is intended to deal with the prompt effect of Shengmai injection on the homodynamic. Swan-Ganz catheter is the golden standard to measure the value of homodynamic. However it fails to find wide application owing to some complications, such as, bleeding, infection and thrombus. The value of noninvasive homodynamic monitoring lies first and foremost in its convenient, low-risk application to a variety of clinical settings. Professors Shoemaker, Feng Wan and Xue zhong Yu tested the correlation between Swan-Ganz catheter and noninvasive homodynamic monitor, and the result showed no significant difference. For this reason, the noninvasive homodynamic monitor is also used here.1.The prompt effect of Shengmai injection on CHF patient.Purpose: Studying the prompt effect of Shengmai injection on CHF patient.Method: 30 CHF patients from March 2003 to April 2003 in Dongfang hospital were treated by auto-control method. Firstly, they received 5% glucose 100ml in 2 hours (50ml/h) as a contrast group, and then they also received 5% glucose 100ml +60ml Shengmai injection in 2 hours (80ml/h) as a treatment group. The noninvasive homodynamic monitor, America spacelab monitor and NBP-4000 blood pressure monitor measured hemodynamic variables.Result: ① 15 minutes later, there was a significant decrease in HR for the treatment group as compared with the pretreatment. The treatment group's SV and CO increased significantly than the pretreatment in 15 minutes,and the time of obvious increase in CI was in 30 minutes. ②There was a significant fall in not only PCWP but also LVEDP on the 5th minute, and the time of significant decrease in TPR was on the 90th minute. ③ The treatment group's PEP dropped significantly in 5 minutes, the time of significant increase in LVET was on the 15th minute, and STR was on the 5th minute. ④The treatment group's blood pressure and oxygen consumption were of no significant changes in statistics .Conclusion: ① Cardiac output increased in a short time , now that Shengmai injection could improve myocardial constriction. ② Since Shengmai injection expanded vessel and increased myocardial constriction, PCWP and LVEDP fell quickly. ③ There was no obvious effect on blood pressure and oxygen consumption. ④ HR fell fast by Shengmai injection.2. The prompt effect of Shengmai injection on the CHF patient deficient in both qi and yin.Purpose: Studying the prompt effect of Shengmai injection on the CHF patient deficient in both qi and yin.Method: Noninvasive homodynamic monitor, America spacelab monitor and NBP-4000 blood pressure monitor measured 17 CHF patients deficient in both qi and yin from March 2003 to April 2003 in Dongfang hospital, and they were observed in the course of the therapy. The drug and research method to be used were the same as those in the previous study. For the sake of convenience, 30 CHF patients were called group A, and those 17 CHF deficient in both qi and yin group B.Result: ①The HR of group B was generally higher than that of group A, and the HR of the treatment group fell significantly in 15 minutes.
