.136 Patients With Lupus Nephritis Tcm Treatment Discussed

Objective:To approch the relationship between the western medical clinic objective evidence and type of TCM syndrome of lupus nephritis, and to approch the idea of treatment of lupus nephritis in TCM that is stumulated by the study from the perspective of integrated TCM and western medicine. Methods:All 136 subjects with lupus nephritis from the affiliated hospital of Hubei College of TCM,comforming to the diagonostic standard,were divided into 4 syndomes accrording to Bianzhen of TCM, and were observed clinic symtomes and some laboratory examinations, and then those symtomes and examinations were analyzed for the correlation between them. Results:l.The analyzed result manifest that clinic classification, hematuria, proteinuria and renal function are correlated with the types of syndomes in TCM: (1). in the type of heat-toxin brimming syndrome, the most common western medical clinic classificaton is nephrotic syndome and chronic nephritic syndome, which account for 93.7% , and their urine test can found hematuria , proteinuria in which, usually, postive number exceed 3; (2). in the type of Yang difficency of spleen and kidney, nephrotic syndome account for 95.1%, and the subjects' urine can only be found proteinuria commonly; (3). in the type of Ying difficency of liver and kidney and in the type of indifficency of both Qi and Ying, latent nephritic syndome are commen, and the former urine test finding can be found that hematuria and proteinuria mostly; meanwhile, the latter urine-Rt can only be found one "+" of proteinuria in most time.(4)renal failure can present in the type of heat-toxin brimming and Ying difficency of liver and kidney.2. According to lupus nephritis active index, value of ESR and C3, self-antibody postive rate in each type,we can see, from the type of heat-toxin brimming to indifficency of both Qi and Ying orderly, activity of illness decreased then by then . decreasing of value of ESR and increasing of value of C3 were noteble in defferent syndome-types in TCM.ANA and ds-DNA's postive rate that indicate the patients's condition descended at various degree accrording the type order in TCM .Among them, difference of postive rate between type of indifficency of both ying and yang and in the type of heat-toxin brimming were noteble (p>0.05), and this result supported the idea that the trend of lupus nephritis active index, that patients' in the type of toxic brimming, yang difficency of spleed and kidney ,ying difficency of liver and kidney are active, and in type of indifficency of both Qi and Ying are inactive mostly, was obvious.3. The hemorheological results indicated the phenomenon of blood stagnation such as thick , sticking, cognitive, collective in patients of each syndome-type in TCM.. Conclusions:With the analysis of 136 cases above and clinical comprehesion,we think the correlation of western clinic classification with syndome-type in TCM is significant to the clinic practice for integrated TCM and western medicine.Lupus nephritis active index, value of ESR and C3 self antibody postive rate indicated the disease's active trend of different syndome-type in TCM, and value of ESR and C3 can be used as anauxiliary tool in bianzhen of TCM;self antibody especially ds-DNA exists in syndome-types of TCM, reflecting the character of inner hot-toxin and the character of blood stagnation in hemorheological results demostrating the importance of clearing away heat and removing toxin. activating blood to eliminate stagnation in therapeutic principle of TCM of lupus nephritis.
