| Objective:To discuss the relationship of differentiation Syndromes of tradition Chinese medition (TCM) with pathlogic of renal ( Lee grading , Ka grade, quantitative score of immunoglobulin degree deposit in glomerular mesangial),blood biochemistry , urine albumen relation of ration in the patients of IgA nephropathy with normal to compensate renal function . Methods:1. 39 cases of IgA nephropathy confirmed by renal biosy with normal to compensate function were come from department of nephrology in the First Hospital of wuhan during November of 2002 and Auglst of 2003 .2. Get rid of 3 cases who are not accord with the selecting standard . 1 is incomplete for materials and 2 are counted too few for small ball of kidney in kidney puncture pathology.3. Accord with standard of differentie symptom of TCM , 36 out of 39 cases were divided into four types:Qi-deficiency of the lung(spleen) and kidney type, Qi-Yin deficiency type ,Yin-deficiency of the liver and kidney type,Yang-deficiency of the spleen and kidney type.4. 36 IgAN kidney pathology carry on Lee grading , Ka grade semi-quantitative score of pathologic of renal biopsy , quantitative score of immunoglobulin degree deposit in glomerular mesangial.5. Observe the biochemical indexes of blood:level of blood urea nitrogen (BUN),serum creatinine(Scr),creatinine clearance rate(Ccr),albumin(ALB),Immu-noglobulin A(IgA),immunoglobulin M(IgM),immunoglobulin G(IgG),compleme-nt 3 (C3) and complement 4 (C4); Urine examination index: quantitative of ptoteinuria in 24 hour.6. Analyse the distribution characteristic with differentiate Syndromes of TCM, Lee grading, immunoglobulin degree deposit in glomerular mesangial and quantitative of proteinuria in 36 IgAN .7. Compare the relations among differential symptom of TCM and Lee pathology grading, Ka pathology grade, biochemical index of blood , quantitative of ptoteinuria in 36 IgAN with normal to compensate function.Result:1. The distribution of TCM Syndrome type In 36 cases of IgA nephropathy with normal to compensate renal function: 9 cases of Qi-deficiency of the lung(spleen) and kidney type, 21 cases of Qi-Yin deficiency type, 3 cases of Yin-deficiency of the liver and kidney type, 3 cases of Yang-deficiency of the spleen and kidney type.2. The distribution of Lee's grades: 0 case of grade I,6 cases of grade II, 14 cases of grade III, 15 cases of grade IV, 1 case of grade V.3. The distribution of immunoglobulin degree deposit in glomerular mesangial:36 cases of IgA (100%),33 cases of cases of C3 (91.7%),27 cases of IgM (75%), 16 cases of IgG( 44.4%).4. Yin-deficiency of the liver and kidney type and Yang-deficiency of the spleen and kidney type case less likely, lose the meaning of counting, so we only discuss the relation between Qi deficiency of the lung(spleen)-kidney type and Qi-Yin deficiency type.5. the comparation of immunoglobulin deposit in glomerular mesangial between Qi deficiency of the lung(spleen)-kidney type and Qi-Yin deficiency type: IgA +IgM +C3 type 7 (77.8%), IgA +IgM +IgG +C3 type 2 (22.2%), no simple IgA deposit in Qi deficiency of the lung(spleen)-kidney type; simple IgA deposit 1 (4.8% ), IgA +C3 type 4 (19%) , IgA +IgM +C3 type 3 (14.3%), IgA +IgG +C3 type 1 (4.8%), IgA +IgM +IgG type 1 (4.8%), IgA +IgM +IgG +C3 type 11 (52.4%) in Qi-Yin deficiency type.6. The comparition between Qi-Yin deficiency type and Qi-deficiency of the lung(spleen)-kidney type in semi-quantitative score :scores of segmental glomrtular lesion showed significant difference (p<0.05). But there is no significance difference (P> 0.05 ) in scores of total ,lesions of gobal glomerulosec-lerosis,interstitial inflammatory infliltration,interstitial fibrosis,tubularatrophy and vascular injury,7.
