Intrahepatic cholestasis of pregnancy is a gestation period unique diseasewhich has characteristic of skin itches and jaundice. It belongs to high dangerpregnancy. At present, the treatment of ICP mainly is Ursodeoxycholic acid,Cholestyramine, Phenobarbital and Dexamethasone et al. The clinical symptoms willoccur again if we stop use the medicine. These medicine usually has side effects if weuse them for Long-term. Traditional chinese medicine has specially adjustmentalfunction for ICP, and it has no obscure side effects. Therefore, decoctionQingdanzishen was drawed up to research its function and possible actionalmechanism for ICP from blood biochemistry value, survival rate of fetal mouse,pathology change of liver, the expression of ICAM-1 on liver, ER on placenta and cellapoptosis of placenta.Part1 Influnce of decoction Qingdanzishen on the blood biochemistryvalue of pregnancy rats, survival rate of fetal mouse and pathology change ofliverObjective: To provide basis of biochemistrial and pathologial experiment by exploringthe effects of decoction Qingdanzishen on the blood biochemistry value of pregnancyrats, survival rate of fetal mouse, pathology change of liver.Methods: The model of ICP rats was induced by EB.All of rats were divided intonormal group,model group, high dose group,middle dose group, low dose group ofdecoction Qingdanzishen and UDCA group at random. Every rat in TCM treatmentgroup was received EB plus different dose of decoction Qingdanzishen until they werein labor. Every rat in western medicine group was received same dose of EB plusUDCA. All of rats were killed when they were in labor.Blood was acquired by pickingthe eyeballs. Fresh liver and placenta were made into light microscope and electronmicroscope specimens. Survival rate of fetal mouse was observed by caesareaning. Thelevel of ALT, AST, ALP, TBIL, and TBA were tested by biochemistry analyzer.Results:The level of every biochemistry value was decreased remarkably in modelgroup than that of normal group (P<0.01).The level of AST, ALP in high dose group was not lower than that of model group (P>0.05),but the level of ALT, TBIL, TBA hadobvious difference with model group(P<0.05orP<0.01). In the improvement of ALT,AST, ALP, TBIL, middle dose group had the best effect, it had no difference withnormal group(P>0.05),in the improvement of TBA, UDCA group had the best effect,middle dose group had no difference in contrast with UDCA group and normalgroup(P>0.05).The survival rate of fetal mouse: Besides high dose group,every treatment groupcould significantly increase the rate(P<0.01).Pathology change of liver: Under the light microscope, the cell structure of liverin normal group was clear, there was no denaturetion and necrosis. In model group, thecells were swelling, gap of liver's sinusoid was very narrow. A few of bile emboluses inthe expanded bile capillaries. In high dose group, the cell structure of liver wasdisorder. Gap of Liver's sinusoid was narrow. But there was no bile embolus in bilecapillary.In middle and low dose group, there had no difference in contrast withnormal group. Under electron microscope, bile capillary in normal group did notexpanded. There was no bile salt in bile capillary. In model group, part of bile capillaryexpanded and themicrovillus in it had lessened or disappeared. In the expanded bilecapillaries, some high density amorphous substances were observed which might bebile emboluses.Conclusion: Decoction Qingdanzishen might be active by improving the bloodbiochemistry value of pregnancy rats and pathology change of liver so as to increasethe survival rate of fetal mouse,which indicates that decoction Qingdanzishen has goodeffect in treating illness and protecting fetus.Part 2 Effect of decoction Qingdanzishen on the ICAM-1 of liver and ER ofplacenta in rats with intrahepatic cholestasis duringpregnaneyObjective: To research the expression of ICAM-1 on liver and ER on placenta so asto explore the possible adjustmental effect and theaction mechanism of decoctionQingdanzishen upon ICP. Methods: The expression of ICAM-1 on liver and ER on placenta were detected byimmunohistochemistry staining.Results: (1)The expression of ICAM-1 in model group was extremely higher than thatof normal group(P<0.01); Every treatment group could drop the expression of ICAM-1remarkably (P<0.01). (2)The expression of ER in model group was extremely higherthan that of normal group (P<0.05); Besides high dose group, middle and low dosegroup could drop the expression of ER obviously(P<0.01).Conclusion: Decoction Qingdanzishen could inhibit the level of ICAM-1 and ERwhich indicates the possible action mechanism upon ICP.Part3 Influnce of decoction Qingdanzishen on cell apoptosis of placenta and thelevel of correlated regulative gene Bax,Bcl-2Objective: To explore possible action mechanism upon ICP of Qingdanzishen bytesting cell apoptosis of placenta and the level of Bax,Bcl-2.Methods: Cell apoptosis of placenta was tested by method of TUNEL and the level ofBax, Bcl-2 was examinated by immunohistochemistrystaining.Results: The number of apoptosis cells in model group was much more than that ofnormal group.Every treatment group could drop the number of apoptosis cells invarying degree, middle dose group had the best effect. The expression of Bax andBcl-2 in model group were extremely higher than that of normal group (P<0.01). Everytreatment group could inhibit the level of Bax and decrease the level of Bcl-2remarkably(P<0.05 or P<0.01).Conclusion: Decoction Qingdanzishen could adjust the level of Bax, Bcl-2,drop theproportion between Bax and Bcl-2 so as to improve ending of pregnancy.
