Objective:Establish model of diversity time limit of Macaca rhesus cerebral ischemia in ordinary temperature,retain brain temperature is less than sixteen degree℃by technology of selectivity ultra-profound hypothermia to perfuse Macaca rhesus brain,investigate protection of Selectivity ultra-profound Hypothermia on Macaca rhesus cerebral ischemia in ordinary temperature.Methods:Fifteen Macaca rhesus is the healthy and adult(provided by Academia Sinica kunming animal research institute) masculinity,sensitive of reaction,normal of nerves function,age from four years to ten years,the mean age is 7.90±1.82 years,weight 4.2-14 kg,mean weight8.22±2.15kg,fifteen Macaca rhesus were randomly divided into four groups,ultra-profound hypothermia perfusion groups after occlusion of bilateral arteria carotis communis 0 minutes(n=4), ultra-profound hypothermia perfusion groups after occlusion of bilateral arteria carotis communis 10 minutes(n=4),ultra-profound hypothermia perfusion groups after occlusion of bilateral arteria carotis communis 15 minutes(n=4),ultra-profound hypothermia perfusion groups after occlusion of bilateral arteria carotis communis 20 minutes(n=3).Incising the sheathes of bilateral carotid arteries,ligating right common carotid artery proximal part,a catheter was distally inserted into right internal cerebral artery(ICA)to connect cold perfusion system,two catheters were distally and proximally inserted into right internal jugular vein(IJV)and right,femoral vein(FV)to connect the lead-in of the device of ultra-filtration and rewarming,a catheter was proximally inserted into left femoral vein to connect the outfa of the device of ultra-filtration and rewarming,a catheter was proximally inserted into right IJV proximal part to monitor central venous pressure(CVP).Clamping left arteria carotis communis and IJV after 0 minutes,10 minutes,15 minutes or 20 minutes, Cool perfusion fluid solution(4.0±0.5℃)was distally perfused into the right ICA to cool the brain,at the same time,two catheters were distally and proximally inserted into right IJV and right FV to reflu the blood of veins,the solution of refluing was perfused the systemic circulation by left FV after ultrafiltration and rewarming.The temperature of brain was maintained 14.1~16℃for 60min,MRI was examinated before and after operation,the vital signs and the hemodynamical parameters were monitored during the experiment,the dysfunctional scales of neurology and MRI were done every week,the brains tissue and other vital organs tissue were examined by light microscopy.Results:Ultra-profound hypothermia perfusion groups after occlusion of bilateral arteria carotis communis 0 minutes(n=4)and ultra-profound hypothermia perfusion groups after occlusion of bilateral arteria carotis communis 10 minutes(n=4)are resuscitate and normal of nervous system,the constructionthe of brain tissue and organ is normal.Ultra-profound hypothermia perfusion groups after occlusion of bilateral arteria carotis communis 15 min(n=4),two monkeys long-term surviving,one monkey physical disability,one monkey deceas,minority of nerve cell died,but normal nerve cell are present.20 Ultra-profound hypothermia perfusion groups after occlusion of bilateral arteria carotis communis 20 min(n=3)anabiosis difficulty,three monkeys are totally dead after the vital signs of Macaca rhesus sustain 3,7,20 hours,generous nerve cell died,nuclear condense and nuclear fragmentation,renal tubular necrosis,myocardial cell hypertrophy. Conclusion:Selective cerebral ultra-profound hypothermia can depress sharply pars encephalica temperature and defend cerebral ischemia hypoxia under ordinary temperature,increase brain tissue tolerance to cerebral ischemia and hypoxia.The gold time is 10 minutes for brain selectivity ultra-profound hypothermia resuscitation,the protective effection on brain gradually degrade when time of cerebral ischemia is over fifteen minutes,the groups of cerebral ischemia 20 minutes do not resuscitate.Part twoThe effection of ultrafiltration to Water-electrolyte balance and blood-volume on model of monkey brain selectivity profound hypothermiaObjective:investigate the change of hematocrit(HCT),blood Glucose,hemoglobin(HB)and Water-electrolyte in ultrafiltration on the model of monkey brain selectivity profound hypothermia clinopodiiherba and resuscitation,demonstrate ultrafiltration maintain model stability in profound hypothermia perfusion procedure,profit rhesus monkey resuscitation.Methods:Fifteen Macaca rhesus is the healthy and adult (provided by Academia Sinica kunming animal research institute) masculinity,sensitive of reaction,normal of nerves function,age from four years to ten years,the mean age is 7.90±1.82 years,weight 4.2-14 kg,mean weight8.22±2.15 kg.Establish hmodel of monkey brain selectivity profound hypothermia,ultrafiltration and rewarming system is composed of bilateral femoral vein proximal part and the right internal jugular vein, cool perfusion fluid from the right internal carotid artery far part—internal jugular vein far part—blood pump—fibro-dialysis apparatus— heat exchange apparatus connected with heterotherm water container bilateral femoral vein duct,establish brain region extracorporeal circulation.Analyze electrolyte t,blood Glucose,HCTand HB at different time(Preoperative,by-pass30min after hemodilution,5min after ultrafiltration end).Apply SPSS12.0 package analysis and mono-factor analysis of variance,test criterion isα=0.05,P<0.05 denote statistical significance.Results:blood sugar obviously heighten post-ultrafiltration and diversify of electrolyte is tiny,hematocrit and hemoglobin degrade sharply post-hemodilution,and obviously heighten post-ultrafiltration, elevate colloid osmotic pressure and decrease water retention.Conclusion: ultrafiltration can Sustain blood volume balance post-perfusion,retain electrolyte balance,decrease hemodilution and tissue edema on model of monkey brain selectivity profound hypothermia,profit rhesus monkey resuscitation post-clinopodiiherba...
