| Objective:Detecting the atrial fibrillation(AF)and paroxysmal atrial fibrillation (PAF)in the patients with and without cardiovascular disease by the method of 12-lead synchronous ambulatory electrocardiogram(12-HOLTER)testing and evaluating the diagnostic value of 12-HOLTER for AF and PAF.Analyzing the relationship between AF as well as PAF and age,sex and cardiovascular disease and discussing the causes of AF and PAF and analyzing AF load data quantitatively to get some valuable-electrocardiac information for PAF diagnosis and treatment.Methods:The data of 2,275 hospitalized patents and outpatients was studied. All of the patients were tested by 12-HOLTER for AF and PAF detection and AF load data analyzing.The data of 12-HOLTER was analyzed with age,sex,cardiovascular disease,and compared with the data of 12-lead ECG and electrocardiac monitoring.Results:(1)By 12-HOLTER testing,364 cases of AF were found,which was 16.00%of all cases.Among the AF cases were 84 of PAF,which was 3.69%of all cases and 23.08%of the AF cases.(2)The detecting rate of AF and PAF by 12-HOLTER was 16.00%and 3.69%respectively,which were higher significantly than that by 12-lead ECG and electrocardiac monitoring(12.79%,12.92%,p＜0.01). The detecting rate of AF and PAF was increased for 3%and 20%by 12-HOLTER.(3) AF and PAF took place more commonly in the patients with cardiovascular disease and mainly in the patients with hypertension,in which were 153 AF,42.03%and 41 PAF,48.81%.AF and PAF were found more in the patients older than 70 years(207 AF,25.06%and 54 PAF,26.09%).The rate of AF and PAF in female patients were little higher than that in the male patients(213 AF,17.47%and 54 PAF,4.43%for female;151 AF,14.3%and 30 PAF,2.84%for male,p＜0.01).(4)Premature atrial beats were observed in many patients with PAF.(5)All of the patients with PAF have some AF load.Conclusions:(1)AF and PAF could be detected and diagnosed by 12-HOLTER. (2)The detecting rate by 12-HOLTER for AF and PAF was higher than that by 12-ECG and electrocardic monitoring and the detecting rate could be improved by 12-HOLTER.(3)PAF and AF took place more commonly in the patients with cardiovascular disease,particularly in the patients of hypertension and coronary heart disease.PAF and AF were common in older patients and female patients.Atrial premature beats appeared more commonly in the patients with PAF.(4)AF load data could be analyzed quantitatively by 12-HOLTER.(5)12-HOLTER could be used to detect,diagnose,analyze and evaluate AF,particularly PAF. |
