Objective:From the basis of establish MODS model of dog, To investigate the effect of continuous veno-venous hemodiafiltration on apoptosis cells of Monocyte in the Multiple organ dysfunction syndrome. Methods:In the present series a standard dog MODS model was established by hemorrhagic shock plus resuscitation and endotoxiemia, these dogs were randomly assigned to either treatment with CVVHDF or spontaneous course. The blood was taked from these dogs at different point after the start of endotoxin injection. Blood samples were taken for evaluation of serum ALT, AST, Cr, urea and blood gas. Fluorescein labeled Annexin-V was used to detect the apoptosis cells of Monocyte and analyzed by flow cytometry. Results:Mean PaO2in CVVHDF groups at 3h(T4),6h(T5),9h(T6),andl2h(T7)after endotoxin injection was significantly highter than in MODS group(P<0.05). The levels of Cr BUN and ALT. AST were decreased significantly in the CVVHDF group compared with the MODS group(p< 0.05). Treatment with CVVHDF significantly decreased monocyte apoptosis ratio in the dogs of CVVHDF group, monocyte apoptosis ratio in CVVHDF groups was significantly lower than in MODS group at T4, T5, T6 and T7 after the start of endotoxin injection(p<0.01). Conclusion:Our results suggest that treatment with CVVHDF effectively attenuated endotoxin-induced hypotension and improved arterial oxygenation in dog MODS model. we believe that pilot investigations are now warranted to better define the biologic and clinical effect of CVVHDF in MODS patients, to open up a new way to critical patients.
