Objective TO compare the difference of plasma Apelin level between essential hypertensive patients and hypertensive patients with heart failure (HF).Methods 120 essential hypertensive patients were enrolled and divided into two groups according to the heart failure, including hypertension with HF group (n=79) and hypertension group (n=41). All patients in the resting state by using standard mercury sphygmomanometer specialist were measured blood pressure three times and got the average, pulse pressure.(PP) Then measuring height and body quality and computing body mass index.(BMI).All patients were fasting 8-12h in the next morning, join 5mL extract cubits venous blood containing EDTA anticoagulation tube, rest for 30min at room temperature, centrifugal take after, on the supernatant fluid℃cross-matched 20.Levels of plasma Apelin and brain natriuretic peptide (BNP) were determined by a sandwich ELISA. Left ventricular (LV),Left ventricular ejection fraction (LVEF) and Left ventricular posterior wall (LVPW) were measured by Philips 5700 ultrasonic apparatus.Results (1) Apelin level in a hypertension with HF group was lower than that of hypertension group (P<0.05)(2) The binary logistic regression showed that age,BNP,LV,LVEF and Apelin were significantly associated with the development of HF in hypertensive patients (P<0.05).(3) Plasma Apelin level of hypertensive patients was higher in the early stage of heart failure than that in the later stage (P<0.05)(4) The plasma Apelin level was lower with the increase of blood pressure in hypertension group, (P<0.05)Conclusion Plasma Apelin level in hypertension patients with heart failure is obviously lower than pure hypertension patients. Its level has a significant relationship with the occurrence and development of hypertension with heart failure. With increasing age, Apelin reduce, left ventricular hypertrophy, low ejection fraction and so on can accelerate hypertension development to heart failure. As one of the index of risk stratification and clinical outcomes Apelin level may be helpful to detect hypertension patients with heart failure.
