Clinical Application Value Of Combined Detetion Of Tumor M2-PK, CEA And CA19-9 For Diagnosis Of Colorectal Carcinoma

Objective:1. To discuss the levels of serum of Tu M2-PK,CEA and CA19-9 in patients with colorectal cancer.2. To discuss the relation between the levels of serum of Tu M2-PK, CEA and CA19-9 and the clinicopathologic stage, Broder degree and lymph node metastasis in colorectal cancer.3. To evaluate the diagnostic value of Tu M2-PK, CEA and CA19-9,as well as the diagnostic value of combined detection with Tu M2-PK,CEA and CA19-9.Methods:45 newly-diagnosed inpatients with colorecatal cancer in the first affiliated hospital of Nanchang university were enrolled as the study group.14 patients with benign colorecatal diseases and 40 healthy adults respectively served as the control groups.The level of tumor M2 pyruvate kinase (M2-PK),carcinoembryonie antigen (CEA),carbohydrate 19-9(CA19-9)in serum were detected. The level of tumor M2 pyruvate kinase (M2-PK) was detected by Enzym-linked immunosorbent assay (ELISA), while the level of CEA and CA19-9 in serum was tested by electrochem-iluminescence immune assay (ECLIA). The clinical references such as clinical pathologic stage, Broder degree and lymphatic mestasis were summarized and analysed. All the data was analyzed by the software SPSS13.0.Results:1. The plasma level of Tu M2-PK in patients with CRC in TNMⅠ,Ⅱ(22.9±8.9U/ml) were higher than those with benign disease (11.2±3.9U/ml) and healthy control people (7.2±1.7/ml),while the levels of serum CEA,CA19-9 was no significant among these three groups;2. The advancer the TNM stage was,the higher the concentration of serum Tu M2-PK,CEA and CA19-9 was detected (P< 0.05).All the differences were statistically significant.4. The higher degree Broder degree was,the worse the diferentiation of colorectal cancer was. And the high concentration of serum Tu M2-PK,CEA and CA19-9 was dectecd (P<0.05)4. With more lymph node metastasis,the higher level of plasm Tu M2-PK,CEA and CA19-9 was detected (P<0.05)5. The diagnostic sensitivity of Tu M2-PK for CRC patiens is 77.8%,which is higher than CEA (53.3%) and CA19-9 (40.0%).The diagnostic sensitivity is getting more potent by combined dectection,combined detection Tu M2-PK and CEA is 86.7%,and Tu M2-PK and CA19-9 is 80.0%.The sensitivity of combined detection of three tumor marker is 91.1%,the specificity is 100%, the diagnostic accuracy is 96.0%.Conclusion:1. Tu M2-PK,CEA and CA19-9 are useful for patients with CRC.2. Tu M2-PK is more useful than CEA and CA19-9 in detecting patients with CRC.3. Tu M2-PK is a potent tumor marker in early diagnosis for CRC.4. The diagnostic accuracy is improved by combined detection with Tu M2-PK, CEA and CA19-9.
